Nice ideal. The reality of healthcare is that (at least) 1 in 20 outpatient diagnoses are wrong (perhaps as many as 1 in 5), and 1 in 1000 orders are for the wrong drug or wrong patient. That’s the level of actual precision that the current system delivers.

When I told folks on TikTok that roughly one per thousand medication orders in the hospital are for the wrong drug, 1 per 1000 prescriptions in retail pharmacy are the wrong drug, and average med error rate in US hospitals is 1 per patient per day, excluding wrong time errors.
