Challenging VT storm with multiple morphologies in a hemodynamically unstable patient for redo procedure. Substrate-guided strategy, focusing on areas of wavefront discontinuity with dual pacing wavefronts suggested a potential VT isthmus. Area corresponded nicely with @inheartmedical wall thinning. This makes sense. When I started learning cardiac anatomy and EP terminology, I thought about TA 4-5 located APs as inferior and septal but I was "forced" to say they where posterior and septal due to stablished convention. 😁. Thanks Dr. @jacabreracardio .
