The number of redo-TAVI procedures is expected to rise substantially as indications expand to younger patients with aortic stenosis who are likely to outlive their index transcatheter heart valve. The ReTAVI registry represents the first prospective, multicentre evaluation of redo-TAVI providing systematically collected, core lab-adjudicated data on procedural feasibility and early outcomes. eurointervention.pcronline.c…

#CTO #PCI skills can help you in a multitude of situations as well! An older patient (80s) presented with NSTEMI and cardiogenic shock, surgical turndown with trifurcation LM disease and ostial LAD CTO. Impella support; LAD identified with #HDR and crossed; followed by DK crush into LAD/LCX and sequential kissing for the trifurcation. I believe that without CTO skills, this patient would likely not make it. #CardioX #ACCFIT

3D vena contracta may add another layer to TR severity grading. @JACohenMD highlights how 3D imaging can better define the regurgitant orifice, and help frame the severity discussion beyond traditional parameters. How are you using 3D imaging in TR assessment? #CardioTwitter

In a 2300-patient RCT, 3-month DAPT was non-inferior to 12-month DAPT (both encompassing ticagrelor plus aspirin, followed by aspiring only) after CABG for saphenous vein graft occlusion and was superior in reducing bleeding risk bmj.com/content/393/bmj-2025… @bmj_latest

ONO save case example. 6-week post-implant TEE showed a WATCHMAN almost out. Transseptal with TruSteer ONO. Captured the device with a Raptor, advanced the ONO to form the basket in the LA, and brought it home. No significant septal injury. #LAAC #CardioX

Historically,we considered Rota relatively contraindicated in STEMI as it can fragment thrombus that is often there, this case shows that experienced operators can sometimes perform calculated higher risk procedures successfully.

🔊 Proper IVL balloon prep matters! Even small amounts of residual air can reduce sonic pressure by up to 20%. And remember: non-ionic contrast alone won’t work. IVL requires a saline-contrast mixture to generate shockwaves. 🎥 See the video for the correct technique.

My Toolkit 🧰⚙️🪛: Self-made steerable catheter to cross the aortic valve in severe aortic stenosis 🔗pcronline.com/Cases-resource… In this article, @Obisht, Won-Keun Kim and Robert R. Flieger describe a self-made steerable catheter system using a 6F EBU guide and the stiff back end of a 0.035″ wire, combined with a hydrophilic straight-end crossing wire, to facilitate crossing of severely stenotic or calcified native or prosthetic aortic valves during TAVI. The technique is particularly useful in horizontal aortas, bicuspid valves, and massively calcified tricuspid or bioprosthetic valves, where the conventional technique repeatedly fails.
