OK - doing this after a long break. Look at the image and see what you think about the red population. Diagnosis?
Don't get to see these often anymore [unlike in the mid-90's when we might see 2-3 cases/week at some centers]. So throwing this one into #PathTwitter. #GoWithTheFlow
Characteristic morphology includes pleomorphism with plasmacytoid/“immunoblastic” look. Once you see a few cytospins, this will tech itself into your brain.
Typical flow is lineage negative (for B/T/myeloid) with presence of “activation markers” like the ones shown (30,71,38..)
Plasmablastic lymphoma is absolutely in the differential for these - similar clinical setting and flow/morphology can be tricky, especially if mass lesions are present (which can happen in effusion lymphoma). Best to keep that differential in mind.
