I talk about 'rare' all the time but this is NEXT LEVEL rare... Throrotrast (colloidal suspension of radioactive thorium-232 dioxide) has been discontinued since the early 1950s!... Discontinuation was driven by recognition that Thorotrast, once injected, was retained permanently in the reticuloendothelial system (liver, spleen, bone marrow, lymph nodes)... with a biological half-life of several hundred years, resulting in lifelong chronic alpha-particle irradiation... German Thorotrast Study: median life expectancy was shortened by 14 years, liver cancer standardized mortality ratio exceeding 12,000–16,000, excess cancer risk persisting for over 50 years after injection... scary stuff!

Non-Langerhans histiocytosis with a constellation of findings you won’t forget once you’ve seen them: 🦓 Symmetric sclerosis of long bone diaphyses/metadiaphyses (sparing epiphyses) 🫘 Bilateral renal soft tissue infiltration → ā€œhairy kidneysā€ on CT šŸ«€ Soft tissue encasement of the aorta → ā€œcoated aortaā€ sign 🧠 CNS involvement — pituitary stalk thickening, cerebellar/brainstem lesions šŸ”¶ Orbital infiltration causing proptosis ⚔ Key associations: → BRAF V600E mutation in ~50% → Diabetes insipidus is a common presenting symptom → 18F-FDG PET/CT is invaluable for staging & treatment response šŸ“ø When you see bilateral perirenal soft tissue long bone sclerosis on a CT — think ECD! x.com/ctisus/status/20523082…
