Across clinical trials and real-world cohorts, pretreatment hypoxia predicted poorer outcomes. In contrast, on-treatment hypoxia-associated TAM programs reflected therapeutic activity. Timing matters: baseline hypoxia and treatment-induced hypoxia are biologically distinct.

Conclusion: ccRCC begins as a pseudohypoxic, vascular tumor. VEGFR-TKIs trigger hypoxia, driving tumor control and recruitment of SPP1⁺ TAMs. However, persistent hypoxia can ultimately promote resistance and mets linking response and failure through the same biological process.

Single-cell profiling revealed distinct immune effects of VEGFR-TKI and immunotherapy. Both remodeled the TME, but ICI preferentially expanded T-cell responses while TKI induced a unique myeloid and B-cell response program, suggesting complementary mechanisms of action.

Next came our main finding: SPP1⁺/GPNMB⁺ macrophages emerge in response to true tissue hypoxia. These cells are rare in untreated tumors but accumulate after VEGFR inhibition, providing a biomarker that distinguishes genuine hypoxia from VHL-driven pseudohypoxia.
