One of the ways we have failed as a society and in healthcare is we have yet to to destigmatize and demystify FND,, conversion disorder, or conversion, or what used to be called hysteria, even though Freud tried to set us on a path to treat it emphatically long ago.

Criticizing the biomedical model of mental disorders doesn’t mean denying that biological factors can play a role in some forms of mental distress. It means rejecting the oversimplified idea that DSM-defined disorders are brain diseases or chemical imbalances, which distorts public understanding. And yes, psychiatrists now claim to follow the “biopsychosocial” model—then they should have no issue with me criticizing the biomedical model because there’s been no real effort to correct public misconceptions. That said, I’m not totally satisfied with the biopsychosocial answer. In practice, the field still treats mental disorders as primarily biological, as its main function remains prescribing medication.

Appreciate it. I did Gabor Mate on a small scale in Trauma Dispatch. He and van der Kolk share an aim of promoting the beliefs most useful to their vision of the world; their aim was never to arrive at the true facts. I've seen many 1000s patients presenting with psych symptoms caused by medical problems/medication side effect or withdrawal/alcohol or substance use. Primary psych diagnosis should never be made til all these are ruled out after careful evaluation & differential diagnosis.
Sure, but let me offer something I see alarmingly often in my clinical practice… Doctors, both PCP’s and psychiatrists, that NEVER ran any blood tests for relatively minor medical issues. Medical issues that will present as a mental health problem. I call them minor because they’re usually fairly cheap and easy to treat. Things like: -Thyroid issues -Low Vitamin D -Low Magnesium -Electrolyte issues in general -a genuine food sensitivity or allergy, many of which come with an atypical presentation The doubly irritating issue in all this is that when I send patients back to their doctors, the doctor tends to be butt hurt that I’m telling them to rule these things out. A not insignificant percentage of these doctors have even gone so far as to leave me rather unhinged voicemails and emails, mad at me and still refusing to do the test. Thankfully most of the labs for a while now will let you get your own blood work done.
