Surprising results from the FRAIL-AF trial presented today at#ESC2023. Switching from VKA to DOAC ➡️ 69% higher rate of bleeding! Is this because the DOAC most used was rivaroxaban? Or do frail patients benefit from frequent INR checks & close monitoring?

The ICS has a guideline on it here ics.ac.uk/resource/cvc-guida… which is endorsed by RCoR and SoR. Ultimately it says it can be used safely (with some qualifying statements).

Existing evidence for TLSO’s is poor. We need real world RCTs involving frail older inpatients with emphasis on outcomes such as compliance, PROMs, delirium, costs and mortality. sciencedirect.com/science/ar…

Bugbear No. 3: The word ‘acopia’ being used to describe a patient's inability to cope with activities of daily living. As Physiopedia notes “This term is disrespectful, implying fault on the part of the patient”. Acopia is a district in Peru, and that’s where the word should stay

May I introduce you to Prior’s Rules? I’ve shared them with many an anaesthetist as they CCT. 1. Remember that “No” is a complete sentence. 2. Always know when your next lot of annual leave is. 3. No one ever said on their deathbed “I wish I’d spent more time at work”.

sapg.scot/guidance-qi-tools/… I was really impressed by this. Was v easy and not very time intensive on the wards. And definitely helped the patient. Also has clear pathway to officially removing allergy notification from all records afterwards.

Fantastic to hear frailty and the @GeriSoc being discussed on the latest Bad People podcast by @SofieHagen and @drjuliashaw, including a sensitive discussion of elder abuse. Important to highlight this as part of #truecrime

Fascinating to hear about techniques of debriding wounds, wound management is not something that I know a lot about, often our nursing colleagues are making decisions on wounds so really enjoying learning from pictures and about techniques today. #bgsconf
