Pain intensity and prognosis of acute pancreatitis in an international, prospective study ➡️ doi.org/10.1093/bjs/znaf155 🔥 Severe abdominal pain at admission linked to worse outcomes in acute pancreatitis (organ failure & necrosis). ⚖️ Moderate pain tied mainly to pancreatic necrosis, but not consistently to other complications. 📉 Pain severity alone was a poor predictor of disease course (low accuracy for risk stratification). ✅ Absence of severe pain may help reassure clinicians and support safe outpatient management in low-risk patients. BJS commission on the surgical management of the axilla in breast cancer ➡️doi.org/10.1093/bjs/znaf125 - The operating room has long been the main place where surgeons in training develop their skills. But in breast cancer surgery, the complexity of modern procedures means this may no longer be enough. - With techniques like ALND, axillary management, nipple-sparing mastectomy, and oncoplastic surgery, the need for highly specialized breast surgeons is greater than ever. - Future training must go beyond the OR: simulators, cadaver labs, hands-on courses, VR/AR, and even AI tools will play a key role in standardizing procedures. - This evolution is not a setback for surgical education — it’s an opportunity to reshape how we train the next generation of breast surgeons. Ultimately, better training means not only improved oncological outcomes but also a better quality of life for breast cancer patients. Predictors of maintained remission at one year following appendicectomy in ulcerative colitis: post-hoc analysis of the ACCURE trial ➡️ doi.org/10.1093/bjs/znaf137 This analysis identifies younger age and former smoker status, both acknowledged as risk factors for relapse, as predictors of greater benefit from appendicectomy in UC patients in remission without advanced medical therapy These results provide a potential basis for refining patient selection criteria for appendicectomy in UC management, although further biological research is warranted to clarify underlying mechanisms Work by Eva Visser , Thomas D Pinkney , Lianne Heuthorst , Geert R D’Haens , Willem A Bemelman , Christianne J Buskens #SoMe4Surgery #MedTwitter #SurgEd #Surgery @RCPSGTrainees @aecirujanos @SEIQuirurgica @iss_sic #MedicalTechniques @BJSAcademy @young_bjs @BJSOpen @des_winter @evanscolorectal @robhinchliffe1 @bplwijn @MalinASund @nfmkok @TejedorPat @paulo_sutt @PVaughanShaw @JJEarnshaw @juliomayol @ksoreide #colorectalsurgery #StepUp4CRC @FightCRC @ACPGBI #ERAS @dice_europe #Crohn #proctology @Dukes_Club @ACPGBI_EduTrain @AECP_FAECP @PelvExGroup @escp_tweets @YouESCP #TeachMeColoproctology #Some4COLoprocto

Ultra-low dose superparamagnetic iron oxide nanoparticle injection for sentinel lymph node detection in breast cancer: prospective cohort study ➡️doi.org/10.1093/bjs/znaf129 🔬 A new magnetic liquid with tiny iron particles helps identify the first lymph node draining a breast cancer. It’s injected into the breast, and the node can be found with a magnetometer, usually in the armpit. 🧪 In this study, an ultra-low dose of this liquid was tested against today’s standard radioactive liquid. Patients received both before surgery. ✅ The very low dose of magnetic liquid worked just as well as the standard. Advantages? No radioactivity, and it can be injected weeks before surgery. Agree. When things get rocky it gets very quiet. Pin drop quiet. When emergency and out of control it can get noisy but the sound level falls with the heart rate and inotrope requirements. Well functioning teams have a level of background noise far below ambient urban or jungle
Interesting. I always thought noise in theatre was a poor metric. I have known surgeons who like a quiet theatre, and others who like chatter and music.
