“& that’s how it’s done, son!” A successful pericardiocentesis performed with my friend and mentor, Dr. Arber Kodra (and our wire even made a perfect ampersand in the pericardium) #Cardiotwitter #Cardiology #InterventionalCardiology

This should be a HUGE story. The insurance companies are robbing patients blind, refusing to pay for the care they’re entitled to, and putting more obstacles for their doctors to provide them evidence based care.

Doctors at a major US insurance company deny tens of thousands of claims a month without even looking at patients’ files. “We literally click & submit,” a former Cigna doctor told @ProPublica & @Capitol_Forum. “It takes all of 10 seconds to do 50 at a time.” The inside story 🧵

“I came in because my wife made me” usually means something disastrous. Great case Dr Fisher! @PerryFisherMD
42 year old man, with a PMHx of HTN and HLD, who experienced chest pain but only came to the hospital at his wife’s behest. Time and again, we are reminded why married men have better cardiovascular outcomes: ahajournals.org/doi/full/10.… #Cardiotwitter #InterventionalCardiology

This could be a HUGE win for physicians and patients- will give even a bigger push to private practice, and to those physicians who want to serve their own communities. This will receive a lot of pushback from the big hospital systems. statnews.com/2023/01/11/ftc-…

Amazing commentary by ⁦@Jcontreras75⁩ about the tragic death of this young woman. Always there advocating for minority communities and those in need! Very proud! This really emphasizes the importance of early detection of congenital heart diseases! telemundolasvegas.com/videos…

Update on Damar: His vitals are back to normal and they have put him to sleep to put a breathing tube down his throat. They are currently running tests. We will provide updates as we have them.

This is a graphic from an @NEJM article which shows that Commotio cordis is when blunt trauma occurs during an appx 20msec Period during the T wave of the ekg causing degeneration into ventricular fibrillation-which is a fatal rhythm if not immediately defibrillated.

In reality, we don’t know what happened, and his doctors probably don’t yet know either. The name of the game at this point is stabilization, and then hopefully when he is through this critical period- he will undergo many many tests for #SCD sudden cardiac death.

Intubation in many of these cases is solely for airway protection, again, the major prognosticator here is “time to ROSC”- or how long his heart wasn’t beating normally for- because this has correlation with neurological recovery!
I am told that Damar Hamlin has been intubated and is currently listed in critical condition. @fox19

This could be a positive sign. It’s not surprising that he could be intubated and requiring some help with breathing, but the major point is if he sustained any neurological damage. Hopefully not.
I’m told Damar Hamlin has a pulse, but is not breathing on his own. He is being transported to UC. Needed AED and CPR on the field.

To me this seems like it is Commotio cordis, in which case IMMEDIATE defibrillation would be the required. Prayers to Damar Hamlin and his family and his teammates. This is tragic.
Damar Hamlin has been down for 9 minutes after this hit. Emergency Personell have been administering CPR. Prayers for Damar Hamlin🙏🏼 #BUFvsCIN #MNF x.com/ABsneurosurgeon/status…
