Really enjoyed hearing from @BruceBiccard and Nicola Vickery at the @GASOC_2015 journal club this evening. They covered design of meaningful context-specific interventions when undertaking research (with specific look @ #ASOS2 trial) #journalclub #globalsurgery

#ASOS1 and #ASOS2 are two of the most impactful studies ever done to inform postoperative mortality #POMR and #periop care across Africa. So fortunate to have @BruceBiccard speaking TOMORROW at #WCA2021 to discuss #ASOS1 and #ASOS2 @AfricanSOS. Join us virtually @wfsawca

A lot of admiration for the work this group is doing. And although I’d hoped to see a “positive” outcome here, by publishing and analyzing their process for us they demonstrate that this is definitely not a “negative” study. We can all learn from this one

Thread describing the process evaluation of the #ASOS2 trial from @BruceBiccard. Changing and measuring care stressed the limited resources of surgical teams in Africa. Co-design of interventions and support of team members key to successful practice change.

6. Implementation fidelity was only achieved in 57% of #ASOS2 intervention hospitals. The intervention (although considered simple and feasible through piloting sajaa.co.za/index.php/sajaa/…) was complex and outstripped the limited resources at the sites. @LeonALIVE

2. In #ASOS2 we identified the patients at greatest risk of death, in order to focus the limited available care to these high-risk patients. It was a hospital cluster randomised trial to provide either enhanced postoperative surveillance to high-risk patients or standard of care.

The high #perioperative #mortality in Africa in #ASOS prompted #ASOS2 to study whether increased surveillance would decrease deaths. It didn’t, but we now have much more data and insight on how to move forward towards saving lives.

I think you will find the #ASOS2 process evaluation paper very informative, which speaks to this point about implementation. It will be online soon in @BJAJournals

Congratulations for such an Effort! Incredible! 160 172 hospitals designating a Regional Research Infrastructure in🌍 > It'd be interesting to see how the research implementation has impacted on the quality of healthcare and, the pt's outcomes. An operational model, indeed.

6. However the #ASOS2 intervention of increased postoperative surveillance did not decrease overall in hospital mortality. (Relative risk 0·96, 95% confidence interval 0·69–1·33; p=0·79)

4. #ASOS2 included 332 hospitals from 28 African countries between May 2019 and July 2020. 160 hospitals (13 275 patients) were randomised to the enhanced postoperative surveillance arm and 172 hospitals (15 617 patients) to the standard care arm.
