We tried to map the current evidence on coil-only MMAE for cSDH, and more questions than answers still! Technique variability is real and there's a lot of room to grow. Hopefully this helps prep the ground for the next wave of studies.

In this JNIS scoping review, authors analyzed 9 studies including 367 patients and 459 embolization procedures evaluating coil-only MMAE for cSDH. Recurrence rates ranged from 0–17.6%, reintervention rates from 0–15.4%, and complication rates from 0–5%. Have long felt that the key reason he invaded in 2014 was because Ukraine signed two huge agreements in 2012 with Shell and Exxon to develop the vast natural gas fields around Crimea and the Black Sea. If Ukraine easy successful in becoming a major producer of nat gas it would have been a major competitor to Russia and hurt its economy. And exporting energy is the huge csdh cow for Putinstan. It also would have been major blow to Putin’s ability to control Europe by shutting off nat gas during the debts of winter. Gas was often restricted by ‘technical problems’ which were corrected as soon as the EU gave Putin what he wanted. There were multiple reasons; a threatening democracy. A very weak Obama/NATO. Ukraine was sbout to join the EU. But having the new fracking techniques at eork next door destroying his main cash cow had to be a major factor.

⚠️Our new study is now published in @SNInt Surgical Neurology International. 👉🏾In this study, we examined postoperative radiographic predictors of recurrence in chronic subdural hematoma (CSDH), focusing on subdural space and midline shift in a single-center cohort. 👉🏾In a series of 428 patients undergoing surgical evacuation, 10.5% experienced at least 1 recurrence. Two postoperative imaging findings stood out as independent predictors, particularly in bilateral CSDH cases: * Midline shift * Size of the postoperative secondary subdural space 👉🏾These findings suggest that routine postoperative imaging is not just descriptive but can directly inform risk stratification. Patients with these features may benefit from closer follow-up and potentially adjusted management strategies. 👉🏾This work contributes to a more objective, imaging-based approach to identifying high-risk patients after CSDH surgery. 🚨 New Results from the EMMA-Can Randomized Clinical Trial! 🧠 Can we reduce the high recurrence rates of chronic subdural hematoma (cSDH)? The newly published EMMA-Can trial evaluated adjunctive embolization of the middle meningeal artery (EMMA) alongside standard surgical drainageacross 9 Canadian centers. 📊 Key 90-Day Outcomes: 📉 Symptomatic recurrence dropped by 6.5X: only 4.3% in the EMMA group vs. 28% with surgery alone. 🧠 Radiographic recurrence was also much lower: 14% for EMMA vs. 49.5% for the control group. ✅ Safety profile: Mortality and adverse event rates were similar between groups, and no serious adverse events or deaths were attributed to the EMMA procedure. 💡 The Takeaway: Performing EMMA with a liquid embolic agent within 72 hours of surgery is a safe and highly effective strategy to prevent cSDH recurrence compared to relying on surgery alone. #Neurosurgery #ClinicalTrials #EMMACan #MedicalResearch #Neurology Thoughts: 1) The data support that cSDH are vascular in origin. cSDH are a common encountered neurological entities with an aging population. 2) At our center we have unpublished data that SDH recurrence after craniotomy SDH evac surgery can be as high as 20% prior to MMA embolization. Recurrence was highest in those on chronic anticoagulation compared to those without anticoagulation. 3) If you talk to a neurosurgeon who does craniotomy and drainage of cSDH, they describe a hypervascular membrane being present that they try to remove as much as possible in a craniotomy flap. This along with some other studies about VEGF elevation support this “cSDH vascular hypothesis” that is treated by embolization. Comment: I once had a paper about subdural hematomas rejected by the typical major journals around this vascular SDH concept. Finally a good journal and reviewer asked “are cSDH really vascular ?” We said yes and provided and cited some early literature around this concept.
