If you see moderate to severe angiographic calcification, always make it a point to image.( fig 1 on how to grade calcification on angio) If IVUS shows: •Superficial calcium •270°–360° arc extending >5 mm or Calcium nodule Atherectomy is required for adequate lesion preparation. Example : Elderly male with recent IWMI, s/p PCI to RCA, now taken up for elective PTCA to LAD. Angiography suggested severe calcification.
