Our latest study examines the heterogeneity of immune checkpoint expression in cancer and its relationship to dysregulated tumor metabolism. While PD-1/PD-L1 blockade, such as pembrolizumab, has demonstrated significant efficacy in melanoma and certain hematologic cancers, its impact in most solid tumors remains limited. We propose that this disparity is driven by immune checkpoint heterogeneity, where multiple inhibitory pathways beyond PD-L1 are co-expressed and may exert equal or greater suppression of cytotoxic T cell function. This layered inhibitory landscape, potentially reinforced by tumor metabolic reprogramming through lactate, may explain the limited responsiveness of many solid tumors to single-agent PD-1/PD-L1 blockade. πŸ‘‡ biorxiv.org/content/10.64898… #cancermetabolism #immunoncology #lactate #immunecheckpoints
