A girl born in Africa today begins her cardiovascular journey long before her first clinic visit. In childhood nutrition. In chronic stress. In environmental exposure. In pregnancy. In obesity, hypertension, HIV, diabetes, inflammation, and menopause. The heart remembers exposure. A hypertensive pregnancy at 28 may become HFpEF at 52. Gestational diabetes may become premature vascular disease years later. Yet women’s cardiovascular care is still too often approached episodically — as though pregnancy, menopause, and cardiometabolic disease are separate conversations. They are not. They are chapters of the same cardiovascular story. The future of women’s heart health must become: longitudinal. sex-specific. prevention-focused. and contextually African.
