“Time in Range म्हणजे नेमकं काय?” ⏳ CGM series मध्ये हा प्रश्न आता खूप विचारला जातो “Sir… HbA1c बघायचं की Time in Range?” माझ्या मते हा प्रश्न थोडा चुकीचा आहे कारण दोघंही महत्त्वाचे आहेत… पण दोघं वेगवेगळ्या गोष्टी सांगतात. 1 HBA1C म्हणजे काय? HbA1c आपल्याला गेल्या २–३ महिन्यांतील sugar ची average (सरासरी) कल्पना देतो. जसं एखाद्या विद्यार्थ्याचं वार्षिक report card पण त्यातून हे कळत नाही: • कोणत्या दिवशी तो आजारी होता • किती वेळा चुका झाल्या • रोजचा performance कसा होता 2 Time in Range TIR म्हणजे काय? Time in Range म्हणजे… 24 तासांपैकी तुमची sugar किती वेळ target range (लक्ष्य श्रेणी) मध्ये होती? CGM हे सतत मोजत राहतं. सोप्या भाषेत: HbA1c = Average Time in Range = Actual daily performance 3 Range म्हणजे नेमकं काय? बहुतेक diabetes patients साठी: 70 ते 180 mg/dL ही target range मानली जाते. म्हणजे: जर तुमची sugar दिवसातील 90% वेळ या range मध्ये असेल… तर Time in Range = 90% 4 मग HbA1c आणि Time in Range मध्ये फरक कुठे पडतो? दोन व्यक्तींचा HbA1c 6.5% असू शकतो. पण: पहिल्या व्यक्तीची sugar: 100 → 130 → 120 → 110 अशी stable असते दुसऱ्या व्यक्तीची sugar: 70 → 220 → 80 → 250 → 90 अशी roller coaster सारखी असते Average सारखा… पण metabolism पूर्ण वेगळा. आणि हाच फरक HbA1c अनेक वेळा पकडू शकत नाही. 5 म्हणून Time in Range ला वाढतं महत्त्व मिळत आहे कारण तो दाखवतो: • spikes किती वेळा येतात • sugar किती वेळ high राहते • recovery किती चांगली होते • body food ला कसा response देतं हे रोजच्या जीवनाशी जास्त संबंधित असतं. 6 Research / संशोधना मध्ये आता Time in Range ला एक महत्त्वाचा metabolic marker म्हणून पाहिलं जात आहे साधारणपणे: 👉 TIR > 70% हे अनेक diabetes patients साठी चांगलं मानलं जातं. 👉 TIR > 90% हे अनेक healthy individuals मध्ये दिसू शकतं. पण numbers पेक्षा context महत्त्वाचा असतो. 7 मग कोणता number जास्त महत्त्वाचा? माझ्या मते… HbA1c आपल्याला मोठं चित्र दाखवतो. Time in Range आपल्याला त्या चित्रामागची कथा सांगतो आणि clinical practice मध्ये अनेक वेळा कथा जास्त शिकवते. आजची Take Home Learning 👇 HbA1c सांगतो: “गेल्या काही महिन्यांची सरासरी काय होती?” Time in Range सांगतो: “दिवसभर शरीर प्रत्यक्षात कसं काम करत होतं?” आणि अनेक वेळा… Average normal असू शकतो पण patterns अजून सुधारायचे असतात. CGM vs. standard care in type 2 diabetes — the numbers are striking. 🧵 RCT data at 32 weeks. CGM didn’t just improve HbA1c — it shifted the entire glucose distribution. Both groups started at HbA1c ~8.8% and TIR ~40%. At 32 weeks: 📉 HbA₁c: 7.9% (CGM) vs 8.4% (control) p<0.05 📈 Time in Range: 60% (CGM) vs 50% (control) p<0.05 📉 Time above range (>13.9): 12% (CGM) vs 21% (control) p<0.05 Significant differences were seen as early as week 16. 💡 Real-time glucose feedback changes behavior — and the data prove it. Your A1C is based on your red blood cells — literally. It measures how much glucose has stuck to those cells over the past few months. The longer the cells live, the more sugar they can collect. But here’s the catch: Not everyone’s red blood cells live the same amount of time. If yours die off faster or live longer than average, your A1C might look lower or higher than your actual day-to-day numbers. That’s why it’s helpful to look at time in range with your A1C — because your meter or CGM shows the real-time story your red blood cells can’t always tell.
