🚨 That swollen lip?
It’s not always “just an allergy.”
Angioedema can turn into a can’t-intubate, can’t-ventilate scenario in minutes. ⏳
Up to 15% develop airway obstruction.
If the larynx is involved, mortality has been reported as high as 56%.
👀 Watch for the red flags:
• Voice change (hoarseness/stridor)
• Drooling 🤤
• Difficulty swallowing
• Anterior tongue swelling 👅
If the voice sounds different — the airway is declaring itself.
When you intervene, think strategy, not speed.
🫁 Awake approach if possible
🚫 Avoid blind supraglottics
✂️ Cric kit open. Neck marked. Backup ready.
Because many emergent cases ultimately require a surgical airway.
And remember — ACE inhibitor angioedema is bradykinin-mediated.
💉 Epi and steroids often won’t fix it. Treat the mechanism.
