❌ PanNET:
• Well-differentiated neuroendocrine neoplasm with an organoid growth pattern.
• Tumor cells show salt-and-pepper chromatin and are synaptophysin positive.
• Can show a sclerotic pattern that mimics the appearance of a glandular lesion.

NOT a metastatic renal cell carcinoma on this liver FNA. The cytomorphology shown here is consistent with metastasis from the patient's known Pancreatic Neuroendocrine Tumor (PanNET). Cytoplasmic lipid vacuoles ("Soap Bubble" cells) are often seen in PanNET (primary and metastatic) and are helpful for the diagnosis.
He had a pancreatic neuroendocrine tumor. These have surprisingly good survivability (if treated properly), about 65 % of the patients diagnosed with a PanNET survive for at least 5 years (compared to 13 % for pancreatic cancer). His hesitation might just have cost him his life.
SEQTOR-GETNE Phase III Trial Updates
This randomized phase III study compared everolimus and streptozotocin 5-FU (STZ/5-FU) as first-line treatment in advanced panNETs, and tested which sequence works best.
🔍 Key findings
• No significant difference in PFS or OS between sequences
• STZ/5-FU achieved significantly higher tumor response (~30% ORR vs ~10%)
• Everolimus provided durable disease control
• Safety profiles were different but manageable
➡️ Clinical message: treatment sequencing should be personalized, not fixed.

Pancreatic neuroendocrine tumours (PanNET) represent the most chemosensitive subtype of NET, with alkylating agents being the mainstay treatment for advanced-stage disease. However, disease progression remains inevitable and presents significant clinical challenges.
This mini-review explores hypermutation in PanNET, with a focus on alkylating-induced changes, and considers the potential of immunotherapy in this context.
