#1330: Sometimes Rotary Glide Path Files Do Better Than Hand Files This patient has been referred by a GP to deal with tooth #36. The preoperative radiograph showed calcification of the pulp chamber and the coronal part of the root canals. As expected, access cavity preparation was difficult and was performed by low-speed burs and US tips. As I always stress, the D canal was first exposed as it’s usually the easy part. The ML and MB were then exposed. Initial negotiation confirmed difficulty. The Chameleon Orifice Opener from @dencoendo was used to flare the coronal part of the three canals. Then canals were negotiated by K files 8 and 10 but could not reach the full WL. Hence, the rotary path file was used to go deeper. However, the K files 8 and 10 failed to reach the apical third. Eventually, I used the S1 Super File from Denco to negotiate the apical third, so the WL is reached as indicated by the EAL. The X ray with the Super File S1 inside the canal shows the extreme S shape in the last 3mm of the ML canal. Also upon flaring the D canal, a second distal canal (the DL) was exposed and explored. Upon reaching the full working length with Super S1 File, all canals were C&S with Dual Shaper 20/04 and 25/04 from Denco. Obturation with the modified hot technique. Then post and core were inserted. #3612: Prepare the MB2 first then the MB1: Why? When the MB1 and MB2 are joint, sometimes it’s easier to negotiate and prepare the MB2 first the MB1 like the presented case. The MB1 curvature was severe and located coronally, hence I always in such cases prepare the MB2 first then the MB1, as long as they are joint. Canals were first negotiated manually then they were flared with the orifice opener (except the MB1). Then they were negotiated to the full WL with Denco rotary path files (PA- Files) size 13/02 and 16/2. Finally, the root canals were cleaned and shaped to size 25 (except the Palatal canal which was prepared to size 30) using Denco rotary files 20/04, 25/04 and 30/04. Obturation was done by the modified hot technique. Procedures were almost the same for the second premolars. #16526: This case was referred to deal with 17. The main difficulties were very narrow canals and S shaped MB canal. Glidepath was prepared first by C file 8 and K file 10, then by @Denco Rotary path files (PA- Files) size 13/02 and 16/2. Finally, the root canals were cleaned and shaped to size 25 for the buccal canals and 30 for the palatal canals using Denco rotary files 20/04, 25/04 and 30/04. It is invasive Shuddha... Dyscohesive, single cells with intact cytoplasm also that one image with cells closely intermixed with fat cells... Invasive it is.
