There’s a difference between gender expression and medical transition — and we need to be honest about it. In this clip I break down why diagnosis matters. If gender dysphoria is a medical condition, then treatment has to follow real clinical criteria. Otherwise insurance won’t cover care, clinicians can’t protect patients, and we end up in a free-for-all that helps no one — not women, not kids, not trans people. You can dress however you want, live however you want. But when it comes to hormones or surgery, there needs to be an actual medical reason. That’s not gatekeeping — that’s how every other branch of medicine works. The HHS Review Authors Were Revealed Today and much of the Internet Is Getting the Story Wrong Again. With today’s release of the long-hidden HHS review authors, the discourse is on fire again. And once again, the loudest voices are the least informed. Since April of this year, @laura_tee_mdgi and I spoke with experts and guests from real lived experience on our podcast TransNormal. Researchers, pediatric psychiatrists, evidence-based medicine experts, rural doctors, parents, and people who transitioned as a child: 1. I’m not a cheerleader for pediatric transition. I’m deeply hesitant. Always have been. Because the honest reality is: you can’t reverse puberty blockers or hormones in a teenager’s body. That’s why I believe: Medical intervention for minors should be rare, extreme, exceptional. Not routine. Not fast-tracked. Not socially incentivized. But that caution does not justify blanket bans. 2. Blanket bans aren’t medical policy - they’re political theater. Bans erase the very small subset of youth who do have longstanding, severe, documented dysphoria and deteriorate during puberty despite intensive therapy. The experts we interviewed all agreed: Some teens benefit, some don’t, and the evidence is mixed. Medicine requires nuance. Politics requires absolutism. We should choose medicine. 3. So where does that leave the HHS review? It was commissioned with a predetermined conclusion. That alone makes it political. But dismissing every concern raised in it is also political. The science is uncertain. The evidence is mixed. The reality is complicated. That’s exactly why we need rigorous standards not prohibition and not denialism. 4. What I’ve learned from the experts we’ve interviewed: - Detransition is real, varied, and not a political talking point (Dr. MacKinnon). - Evidence quality is low because rare conditions are always studied poorly (Dr. Guyatt). - Real-world clinics do far more screening and caution than social media claims (rural family doctor parent). - Kids don’t come in as blank slates and neither do adults (our expert panel). All of this points to one thing: Nuance is the only ethical path. 5. That’s why I drafted a starting point revised WPATH SOC called SOC 2025: a rigorous, conservative model that protects kids and trans people. SOC 2025 calls for: - DSM-5 confirmed dysphoria - Full mental health stabilization (12 months) - Documented persistence (24 months for youth) - Trauma/autism/peer influence ruled out - 3–5-specialist verification - IRB oversight public outcome tracking - Zero medical intervention pre-puberty - Blockers/cross-sex hormones only in the rarest cases This isn’t “affirmation-only.” This is medicine with guardrails. 6. We also need legal guardrails - not ideological ones. That’s why I drafted a starting point at a new legal framework called the Secure Gender Recognition Act (SGRA 2025): - No self-ID - Background checks - Clean-record requirement - Psych evaluations - Probationary gender marker - Revocation for violent/sexual crime - Protection of single-sex spaces - A legitimate path for trans adults This aims to protect women, children, and trans people without the chaos of current laws or the cruelty of bans. 7. Why I care Because my own life was turned into a political weapon. Because I’ve met the families living this fight for real. Because I know what it feels like when policy forgets the human in the middle. NO MORE ALL OR NOTHING APPROACHES. I’m advocating for careful, transparent, accountable, evidence-based systems that protect everyone involved.
