The main pre of X has been the size of the medical community. But medical bluesky is gaining rapidly. Both #medsky and #cardiosky are now alive with discussions, and I meet more and more friends from cardiotwitter, but not leaving X yet. You'll find me at @strain-rate@bsky.social

BlueSky is still small and a bit of a bubble. But there are many good reasons for being on BlueSky too.
It feels almost exactly like twitter but under the hood it has some substantial improvements.
