In honor of #Ramadan being 1 month away, I will be compiling a series of tweets in this thread reviewing the available literature and recommendations for pts with chronic diseases who wish to fast the month of #Ramadan This endeavor was inspired by a clinic patient of mine 🙏🏼 1/
Educate pts on the following:
🚩Break the fast if 👉🏼
⬆️in weight (4lbs from baseline) or Sx of volume overload
⬆️SCr by 30% above baseline, hyperK, hyperPhos, hypoNa
❌Avoid foods high in K Phos when breaking fast (dates, dried fruits, nuts, cheese, fried food, deli meats, soda)
🔹Monitor pts by labs at least x1-2/week
🔹Discuss risks & benefits with pts, devise a tailored management plan if they are adamant about fasting
🔹Rehydrate w/ 1-2.5L of💦when breaking fast but do not exceed amount
🔹if tendency towards hyperK, take calcium Resonium or lactulose
