Severe (and isolated) TBI patients do not do well if hypotensive post intubation. Higher SBP thresholds needed to prevent mortality than previously published.

Prehospital publication alert!! Hugely proud to have delivered this publication on the morbidity and mortality associated with post intubation hypotension. This redefines the blood pressure targets for intubation in TBI, associated with outcomes. jamanetwork.com/journals/jam…

Amazing work by @ehaatClinical Nuanced and expert care where the ALS guidance refers to ‘seek expert help’ - lots of building of important OHCA innovation by this organisation/team.
…..more cardiac arrest work. This time nuancing arterial access during arrest so we can focus on targeting pressures, chest compression fraction and so on, to hopefully achieve more survivors. Dream team @EHAAT_ @ehaatClinical PHCC

The compassion bag. We take plenty of equipment to the scene - compassion is perhaps the most important thing we bring…..@Kershberg @ehaatClinical Proud to say that I think we have some of the most compassionate clinicians in the world attend to our patients @EHAAT_

Another publication demonstrating the cost effectiveness of ECPR. Lots of heterogeneity but this demonstrates ECPR is the future of OHCA for selected patients. Requires strength, leadership and the pursuit of better outcomes for patients to deliver resuscitationjournal.com/art…

These are 2 amazing research leaders. Researching the pre hospital space is extremely hard. Investing in research infrastructure & talent is essential for for HEMS charities to deliver evidence based improvements for the patients that fund them. Well done EHAAT!

After 5 and a bit years as Deputy Medical Director at EAAA I’m excited to start a great new job as Deputy Chief Medical Officer at the James Paget Hospital. I’ll use the leaving gift I received from EAAA to remind myself that closing one door can lead to opening a bigger one 😁

ECPR improves outcomes in refractory OHCA, but most patients never reach an ECMO center in time. 📍New systems are expanding access by initiating ECPR in the prehospital 🚑🚁 or non-ECMO center setting. But what happens after cannulation? 🧵👇 (1/5)

Arterial cannula for percutaneous femoral VA #ECMO: relationship between cannula size, predicted arterial size & vascular complications 🧮 median predicted CFA diameter 5.4mm, 1.9 Fr lower vs chosen cannula ⚖️ significant discrepancy = higher complications Estimated CFA diameter should be considered at cannulation, as well as target blood flow to support metabolic demands. 🚨 In just 2.5 months since launch, RescQ has registered over 100 bystanders involved in Out-of-Hospital Cardiac Arrest (OHCA) events. We're proud to be the first UK (at least!) to document the bystander denominator & provide with much needed support.🧵 @DrKeeble @EastEnglandAmb

I was highly skeptical that there was a benefit to recognising and treating occult VF diagnosed with POCUS versus traditional electrocardiographic (ecg)VF This has made me revise my preconceptions. There might be a benefit to diagnosing occult VF with US and treating it as such
Are you missing occult VF ( VF not recognised on ECG but visible on echo) in cardiac arrests masquerading as asystole OR even PEA? Check out the Reason 3 cardiac arrest trial and don’t forget to diagnose, defibrillate, and even consider ECPR in these pts.doi.org/10.1016/j.annemergme…
