Important new article Based on 37 publicly available detransition testimonies, the authors describe pathways into and mechanisms of maintenance of trans identity, as well as catalysts for detransition and post-transition concerns. Despite the paper's limitations, which the authors acknowledge (non-representative sample, subjective self-assessment, etc.), it should serve as a touchstone for future research on etiology, persistence, and detransition. Oregon's use of cross-sex hormones (CSH) for youth gender dysphoria among 14–15-year-olds runs about 4–5x the national rate. By age 17 the gap narrows to ~2–3x, even as the absolute rate climbs with age to about 1 in 240 natal girls and 1 in 630 natal boys having received CSH. /1
Over 1 in 250 Natal Girls Were Taking Testosterone By Age 17 in Oregon, From 2016 to 2023 open.substack.com/pub/benrya… This rate, from a study of data about insured adolescents in Oregon, represents an average over this period. Given swift increases in hormone uptake over time, the rate was likely much higher by 2023. The use of gender-transition drugs among adolescents in Oregon soared between 2016 and 2023, charting a nearly 10-fold increase among biological girls and a 14-fold increase in natal boys. Across this period, about 1 in 100 insured youth ages 8 to 17 in the Pacific Northwest state were diagnosed with gender dysphoria, a psychiatric condition involving distress stemming from a conflict between an individual’s natal sex and their gender identity. By age 17, about 1 in 240 insured natal girls were taking testosterone and about 1 in 630 natal boys were taking estrogen in Oregon. These figures are nearly three-fold higher for natal girls and nearly twice as high for natal boys as figures from a study published last year that analyzed national data on commercially insured youth from 2018 to 2022.

I was there because I believe the proposed law could silence the one kind of help that could have saved me from years of anguish and a future permanently marked by what was done to me as a child. I grew up in a fundamentalist Christian town in rural Michigan with 2,500 people, one red light, and a pervasive drug problem. My parents split when I was a baby: My biological father was a deadbeat I’ve met only a handful of times; my mom worked constantly as the primary breadwinner for our family, building multiple small businesses in the car world. From my earliest awareness, I knew I was not like other kids—and certainly not like other boys. I moved and spoke in ways others called “girly.” I loved dress‑up games, butterflies, and anything pink. I was obsessed with The Princess and the Frog and looked up to Disney princesses more than any male character. I also knew, from a very young age, that I liked boys. The label has been used to shut down scientific inquiry, intimidate academics and clinicians, mislead the gender public, and foreclose thinking. New from me in @unherd: When gender medicine crashed out Secret WPATH videos open a woke time capsule My very presence here as an indigenous person contributes to and reinforces settler colonialism.” So declared Charlie Amáyá Scott, a presenter at a gathering a few years ago of the World Professional Association for Transgender Health, or WPATH. Yet despite fears about reinforcing colonialism, Scott touted WPATH’s larger mission: the promotion of modern Western gender medicine to address the traditional “two-spirit” identities prevalent among some traditional Native American communities. Scott’s presentation came from a batch of nearly 500 videos of conferences held by WPATH that I recently obtained. It’s a motherlode of material on pediatric gender care that the secretive activist medical organization had long fought to keep from the public. The tapes, subpoenaed by the Alabama attorney general in response to a federal lawsuit, covered a pivotal period from 2021 to 2023, when pediatric gender medicine first began to face political backlash. In addition to egregious lapses in judgment on the part of WPATH, the videos are a case study in the excesses of peak woke, neatly preserved, as if in a time capsule.  During a long winter of dyspepsia, I digested well more than 100 hours of the academic faithful checking their privilege and preaching about “centering lived experience.” And I can now report that there are few subcultures more slavishly faithful to the woke religion than WPATH was during the post-Racial Reckoning micro-era.  Link: unherd.com/2026/04/when-gend…
