🫁 ICI Rechallenge in ES-SCLC.
In a real-world study of 151 patients progressing after first-line chemoimmunotherapy, ICI rechallenge improved ORR (26% vs 11.8%), PFS2 (3.98 vs 2.43 months), and OS (9.43 vs 6.23 months), with low rates of severe irAEs (2%).
Prospective validation is needed, but these findings support reconsidering immunotherapy in selected patients with relapsed ES-SCLC.

‼️ Landmark #lungcancer research published today in Cell
Scientists at the Francis Crick Institute and UCL have discovered a 14-protein blood signature that predicts lung cancer more than 5 years before diagnosis, works in never-smokers, and could identify who benefits from prevention treatment.
People with high levels of these proteins were the ones who actually benefited from a drug to damp that inflammation down, cutting their lung cancer risk nearly in half.
This could finally give us something lung cancer has never had: a way to prevent it in the people most at risk.

🧠 Brain metastases and trial eligibility in lung cancer.
Despite BM occurring in up to 50% of NSCLC and 80% of SCLC, only 11.4% of lung cancer trials allow patients with active, untreated BM.
A proposed GPA-based framework could safely broaden eligibility, improve trial generalizability, and expand access to novel therapies.
Time to move beyond automatic exclusion.

The primary is the source of first progression in >50% of cases of #lungcancer patients, and TRACERx showed us that in 32% of NSCLC the primary polyclonally seeds metastases. In our @IASLC consensus, now in JTO shorturl.at/GzAe2, makes the case for eradicating it with #radiotherapy. The EGFRm phase III data are the most compelling OS 34.4 vs 26.2 months with TKI RT. What does chemo add to immunotherapy in pts w/ PD-L1-high NSCLC?
In our meta-analysis of 24 trials w/ reconstructed individual patient data, we observed a meaningful improvement in both median PFS (11.3 vs 6.8 months, HR 0.74) and OS (29.2 vs 19.8 months, HR 0.67) w/ chemo-ICI.
