✅ Correct answer: C) Malaria Artemether Lumefantrine is an ACT (Artemisinin-based Combination Therapy) used for the treatment of uncomplicated malaria, especially caused by Plasmodium falciparum. Why this combination? Artemether → rapidly reduces parasite load. Lumefantrine → longer-acting drug that clears remaining parasites and reduces resistance. It is one of the WHO-recommended first-line therapies for falciparum malaria in many regions.

✅ Correct answer: B) Liver cirrhosis with ascites The combination of: Spironolactone 100 mg (aldosterone antagonist, potassium-sparing diuretic) Furosemide 40 mg (loop diuretic) is classically used in ascites due to liver cirrhosis. Why this combination? Cirrhosis causes secondary hyperaldosteronism, leading to sodium and water retention. Spironolactone counteracts aldosterone and is the primary drug for cirrhotic ascites. Furosemide enhances diuresis and helps maintain fluid balance. The 100:40 ratio helps maintain potassium balance: Spironolactone → retains K⁺ Furosemide → loses K⁺

Ig this would actually shock Dr Gregory House😅
Surgeons once operated so fast people died in the audience. Before anesthesia and antisepsis: surgeries were done at lightning speed screaming was normal. infection was expected In 1847 Robert Liston, a Scottish surgeon from the UK, was famous in the pre anesthesia era performed an amputation in 25 seconds, operating so quickly that he accidentally amputated his assistant's fingers as well. Both later died of sepsis & a spectator reportedly died of shock, resulting in the only known procedure with a 300% mortality rate. A “300% mortality surgery.” #MedX

One major reason doctors face abuse from patients and attenders Some seniors humiliate juniors in front of patients just to display authority. That resident or intern is the same person staying in the ward for 12–36 hours, managing everything for those patients. If you publicly insult your juniors, what respect do you expect patients or attenders to have for them afterward? Correct mistakes privately. Humiliation helps nobody.

I loved the show, damn so much But only the initial 2 or 3 seasons when it was more focused cases and on the story line but later it changed its course, became very much story driven.

Your thread reminds me of a particular scene from House MD, where the House advises his patient to smoke.

The correct answer is B) Septic shock. Why? This patient has classic features of septic shock, a form of distributive shock caused by severe infection leading to systemic vasodilation and tissue hypoperfusion. Key clues: Fever → suggests infection Confusion → cerebral hypoperfusion/sepsis-associated encephalopathy Low urine output → poor organ perfusion Hypotension tachycardia → shock state Warm extremities → characteristic of early (“warm”) septic shock due to peripheral vasodilation Elevated lactate → tissue hypoxia/hypoperfusion Definition According to the current definition, septic shock is: Sepsis with persistent hypotension requiring vasopressors to maintain MAP ≥65 mmHg and serum lactate >2 mmol/L despite adequate fluids. Why not the others? A) Cardiogenic shock → usually cold clammy skin, pulmonary edema, raised JVP C) Hypovolemic shock → typically cold extremities, history of fluid/blood loss D) Anaphylactic shock → usually urticaria, wheeze, angioedema, allergen exposure So the presentation best fits septic shock.
A 70-year-old man presents with: • Fever • Confusion • Low urine output On examination: • Hypotension • Tachycardia • Warm extremities Labs: elevated lactate What’s the diagnosis? 🤔 A) Cardiogenic shock B) Septic shock C) Hypovolemic shock D) Anaphylactic shock @3DMedico1 @AbhishekUp52351 @ChShafiullah15 @ajlkfad2333 @daoo100 @docakx @dermatology @DocPriyamMD @Doctors_GUILD @Dr_Shiv_kumar_ @drabdulhameed07 @DrArslan480 @drkeithsiau @DrMedica_13 @drobiy12 @drsthanus @drxmelodyy @EricMbuyamba @hemo_shk @PharmaFactz @nervemed @TrackYourHeart @DoctorMed0 @Doctors_GUILD @ZahoorAhmed1450

The most common indication for Meropenem is the treatment of severe hospital-acquired infections caused by resistant bacteria, especially: Complicated intra-abdominal infections Sepsis Hospital-acquired pneumonia / ventilator-associated pneumonia Complicated urinary tract infections Meningitis (especially bacterial meningitis) Clinically, it is very commonly used when infection with multidrug-resistant Gram-negative organisms is suspected, including: Pseudomonas aeruginosa ESBL-producing Enterobacteriaceae Meropenem is a broad-spectrum carbapenem antibiotic with strong activity against many Gram-positive, Gram-negative, and anaerobic bacteria. It is usually reserved for serious infections to reduce antimicrobial resistance.
