Over 50,000 people in the U.S. die from pancreatic cancer every year. After this drug is approved and widely used, that number will remain essentially the same. In absolute terms, they are reporting a median survival shift of around six months. Yet we know resistance inevitably develops, as it does in all cancers subjected to drugs targeting mutations in the RAS/MAPK/PI3K pathway. If the goal is to meaningfully reduce cancer mortality, this does not move the needle. This is where decades of focus and billions in NIH/NCI funding have concentrated. National Cancer Institute is funded at roughly $9 billion per year, and a substantial portion of that budget is devoted to oncogenes and what is marketed as targeted therapies. This is then layered on top of a drug development and healthcare model where drugs like this can cost over $100,000 per patient. These are incremental gains at the late metastatic stage, where the biology is already stacked against you. Meanwhile, the two areas that actually determine population-level outcomes—early detection and prevention—remain neglected. If we are serious about reducing the number of people who die from pancreatic cancer, the priority cannot be continuing to optimize late-stage interventions that predictably yield temporary gains. The goal should be zero deaths. Right now, we are not on a path that gets us there. It is not surprising that this view is being met with backlash. Much of the criticism is coming from people whose incentives—academic , financial, or institutional—are tied to maintaining the current system in biomedical research and the biotech and pharma sectors that profit from it.

If the election outcome is as I expected, it should cause the large minority of the country who supported @KamalaHarris and predicted her victory to begin to question their sources of truth. Half the country has believed that @X is filled with mis- and disinformation, and that they could only therefore rely on The NY Times, MSNBC, CNN and other mainstream media for their news. And they did. If, however, you have been active on @X for the last year, you have known the truth days, weeks and often months before the facts appear in the MSM. The MSM excerpted, clipped and cut to defame @realDonaldTrump while claiming that @JoeBiden was fit as a fiddle. Then when Biden’s polls collapsed, @KamalaHarris was anointed the candidate and her hagiography was written with glowing acclaim from the press. But this could not hold as she ducked the media and held fast to the teleprompter. Citizen journalists with their phone cameras in hand captured the real Kamala forcing her to defend her record and her plans in more media appearances. It did not go well and the public demanded to learn more so @KamalaHarris had to risk more unscripted media. The doom loop was underway with perhaps 60 Minutes as one of the more dramatic examples, even after CBS tried to save her, most glaringly by excerpting one answer to replace a word salad response to another. But the citizen journalists on @X quickly caught and outed this fraud and demanded a transcript. As many who supported Kamala began to realize that they have been misled, they became open to Trump as an alternative, but they didn’t want to rely on the media to understand him because they did not want to be misled again. They wanted to hear the candidate in his own words and that is where @lexfridman and @joeroganhq long form podcasts came to the rescue. When Kamala was offered the same opportunities to explain herself, she rejected them. And the voting public could only draw a negative inference. When the story of this election is written, I expect it will be as much about how half of America woke up to the reality that they have been manipulated by the media. This should lead to an abandonment by many of the MSM as their primary source of information. It will push more people to @X, to podcasts and other empirical sources, and it will lead to a more informed public. The other outcome I hope happens is the implosion of the Democratic Party. The Party lied to the American people about the cognitive health and fitness of the president. It prevented, threatened, litigated and otherwise eliminated the ability of other candidates for the primary to compete, to get on ballots, and to even participate in a debate. The Party and the administration used lawfare in an attempt to imprison, bankrupt or otherwise kill off Trump as a candidate. These acts are collectively grave threats to our democracy. With the highest irony in order to hide these acts, the Party accused the opposition candidate of being the grave threat to democracy. The Democratic Party proved itself to be fundamentally undemocratic. It needs a complete reboot. The leadership should be thrown out and those responsible should apologize to the American people. Honest Abe said it best: You can fool some of the people all of the time, and all of the people some of the time, but you can not fool all of the people all of the time.
I believe that @realDonaldTrump will win in a decisive victory and potentially a landslide Electoral College outcome tomorrow. I could of course be totally wrong, but that helps elevate an important point. Approximately half the country will be unhappy about the election outcome, potentially devastatingly so. In this context, I think it is important to remember that we are one country, and we will survive whoever is our next president. What we must avoid, however, is a world where our fellow citizens who are disappointed raise hell or otherwise revolt about the outcome. The peaceful transfer of power is a critical feature of our democracy that must be maintained for us to succeed. The system is stronger than any one candidate. We will thrive long term no matter the outcome of this election because the system will heal itself. That’s been a feature of our democracy for nearly 250 years and I expect it to continue. Let’s therefore be civil and accept the collective will of our democratic process. We have many enemies that want internecine battles in America to weaken us. We cannot let this happen. The world is a dangerous place and we need to be unified to protect our country and our children.

As much as last night was an indictment of the Democratic Party for misleading party members and the country about the mental acuity and health of the president, the media deserve far more derision and scorn. I and others were repeatedly criticized by the media for questioning the competency of the president. Among other false accusations, I was accused of spreading misleading videos which clearly showed Biden’s deterioration. Do you remember the heavily excerpted and edited @POTUS Biden @60Minutes interview where the interviewer covered for the president by saying he was ‘very tired?’ @60Minutes knew. The @nytimes knew. @CNN knew. @MSNBC knew. Left wing media have had total and complete access to the president, his staff, and his administration. They all knew, but they told you otherwise. They outright lied to you. When Robert Hur, the special counsel who deposed the president, said that the president was not fit to stand trial and therefore chose not to bring charges, the media described him as a tool of the Republican Party and character assassinated him. When the @WSJ recently published a several-thousand-word, carefully researched, front page piece on the president’s mental and physical health, it was described by left wing media as outright propaganda. Now consider who has been feeding you propaganda. A favored technique of some of the most evil leaders in history was to mislead the people by constantly repeating the Big Lie. The Big Lie is so audacious that people accept it as truth because it is repeated so often that how can it be that something so important and material could be an outright falsehood? In this case the Big Lie was our president’s fitness for office, let alone a second term. A media organization is not supposed to be a branch of the Democratic Party. The media have a profound obligation to tell the truth to the American people, particularly about something as critical for the country as the president’s mental and physical health. People very close to me, my closest family and friends, trusted the media on Biden until the @CNN commentators finally owned up to the truth about Biden last night. For months I have been accused by many friends and family of being misled by an @X-based ‘right wing echo chamber.’ The sad reality is that one of our most important institutions, the so-called ‘Fourth Estate,’ fourth only after the clergy, the nobility, and the commoners, has destroyed any remaining credibility it has. Consider how your perception of @POTUS Biden and @realDonaldTrump has been manipulated. The media can no longer save itself. A suggestion. Rely on empirical data as much as possible. Listen to what someone actually said, rather than a headline summary or article about what someone said. Get your news from people who have a track record of telling the truth, people who do so at significant personal cost like whistleblowers. I follow broad constituencies on @X on multiple issues. That has led me closer to the truth. Citizen journalism has been a much more accurate representation of reality. Thank you @elonmusk for saving this platform. It gives us a fighting chance to save our democracy.

I have been criticized by some for sharing these @POTUS Biden videos which appear almost every day. I have read numerous articles which somehow suggest that the camera angle, the duration of the clip or some form of right wing manipulation explains Biden’s behavior on camera. Some people are old at 81 and others are sharp, vigorous, and impressive. Compare Warren Buffett at 93 and Biden at 81. Biden is an old 81 who can’t find his way off a stage or stay present and focused at a G7 conference. Is that who should be the leader of the free world for the next five years? A president should not have to be led off stage by hand or with an arm wrapped around him. That is the not the image of strength and leadership we need as a country. Look at what is going on in the world. The perception of weak leadership in the United States has led to global chaos. The reality of weak leadership is a long-term serious and continued threat to our country. The Democratic Party is destroying itself by advancing Biden for a second term. This is the Emperor’s New Clothes in real life. But it is not a children’s book or a joke. The world is at great risk, and a Biden second term is a grave threat to global security and prosperity.
