“I sit on a man's back choking him and making him carry me, and yet assure myself and others that I am sorry for him and wish to lighten his load by all means possible… except by getting off his back.” -Leo Tolstoy, “What Then Must We Do?”
The Trump administration is quietly moving to change the Affordable Care Act to let health insurers offer people loans to pay for their care. Deep in a document over 1,000 pages long about how the Affordable Care Act market will operate next year, the administration suggests that insurers consider offering loans to cash-strapped customers. Under this approach, people who develop a costly disease or need unexpected emergency care would turn to their health insurer for loans. A third of American households already have medical debt, and this approach would mean even more debt that patients owe to their health insurance companies. The insurers, who already make billions, would stand to make even more. nytimes.com/2026/06/11/busin…

This standing ovation totally brought me to tears. On Thursday, my dad (stage 4 pancreatic cancer) started a Revolution Medicine trial building upon these results — RMC 5127, which uses the drug celebrated here, Daraxonrasib, with another drug targeting his specific KRAS mutation, G12V. He’s nearly 2 years into his fight with pancreatic cancer and we have personally felt the impact of this watershed moment. It is a lifeline. To the incredible, stalwart, brilliant researchers and folks bringing these discoveries to life - thank you. Thank you thank you thank you.
One of the most amazing things I’ve ever seen: a standing ovation for the full Daraxonrasib results I feel inspired and energised, to put it mildly — we have a targeted therapy for pancreatic cancer now, and nothing is undruggable anymore

He’s Mr. T, not Dr. T, but the message still applies! Happy Mother’s Day 🙏❤️
Mother's Day - courtesy of Mr. T. I pity the fool who doesn't honor his mom today.

I glance down at the paperwork my patient has filled out before his visit. Occupation: Retired harbour master. Two things: I’ve never met a harbor master before. And “harbour” with a “u.” Interesting. It’s the little everyday mysteries that I love most. That reveal us. 1/

I really enjoyed sitting down with Dr. Saba Fatima @SabaFatimaAli and having this conversation. I honestly wasn’t sure which way things would go, or what exactly we’d end up talking about, but sometimes the best conversations start that way. youtu.be/rJzuKf-57iU

Hey @DGlaucomflecken, the Internal Medicine goal of Perpetual Rounds is coming and there’s nothing you can do to stop us. Just need my trusty Foley, and backpack snack fridge, and I’m gonna round until notes have been written on the entire city. Bwahahahaha! #RoundsR4Evr
careful: if this technology falls into the hands of internal medicine docs, rounds may extend to 8 hours long

Hopefully this isn’t needed but are there a few people in the #Medicine community who students who don’t match can contact? I applied #EM I’m hoping for the best but preparing for #soap just in case. @blondemedSJW @embasic @EM_RESUS @TheRealDoctorT @ChaseTMAnderson

Had a blast spending time with some 6th-8th graders, teaching them about the kidneys. These kids are so talented. Some made posters, combining their love of art and science. I had to help judge them to pick a “winner.” Talk about an impossible task! The future looks bright…

RIP James Van Der Beek from colorectal cancer. Little side story. One of the nurses I work with, loved Dawson’s Creek so much she named her daughter Dawson. In 2020 I asked James to record a little message for them both (via Cameo). 1/2

He could have spent 10 seconds on it but he was gracious and kind. Wishing his friends and family peace. Please remember to get your colorectal cancer screening done beginning at age 45 if you don’t have specific risk factors (check with your doctor). 2/2

They always said the future was going to be what we made it. I just never imagined ... this is where we were headed. It’s 2076. I am ninety-five years old, and living in the Allied Territories of Greater America. The States stopped being “United” long ago. Long ago. 1/
