If a paramedic asks you if they can intubate your patient they brought to your ED who needs the procedure….the answer should always be yes

It goes with out saying, any procedure outside their scope of practice, like awake intubations etc, do not qualify.

If you disagree with my opinion, I encourage you to still allow them to be involved in the procedure. BVM ventilation is such a crucial life saving skill that needs practice and these can be powerful teachable moments.

Replying to @WHOOP
@WHOOP Very disappointed. You said there would be a lifetime membership for founding members for 3.0 to not upgrade to the 4.0 and take the subscription fees, and now are saying there is not. This is essentially the same deal as if we had upgraded to the 4.0 over a year ago.

So lost out on over a year of the “new technology” and being forced out of our founding members promise anyways and being gifted a free band as compensation? Pretty lame

If you see heat stroke patients in your ED, make a protocol. If you do not, you probably will soon enough.
Fantastic hyperthermia protocol buried in the letters of this month's Annals. Best practice translated to logistics-heavy, granular detail broken down by role and localized to department specifics. Bravo @bfeiny et al. annemergmed.com/article/S019… emupdates.com/hot

We started doing this in the prehospital arena with Mehlville Fire in St. Louis and I’ve since incorporated it into my personal practice @WUSTL_EMS
Many patients initially diagnosed as PEA might actually be in shock. Instead of using your fingers to check a pulse, use POCUS! Every five second delay in pulse check increases mortality by 18% @HedayatiMD @CriticalCareNow #ResusXROSC

For anyone considering an EMS fellowship, @WUSTL_EMS, should be on your short list. I never expected to live in Missouri, but fortunate to discover it’s program with innovative and world class EMS systems. And surprisingly really good mountain biking trails /community as well.
We are so proud of our graduating EMS Fellows Dr. Charles Smith and Dr. Bryan Feinstein. You have made a positive impact in the care of patients in the prehospital setting. You are forever a part of the WashU EMS family! @bfeiny @charliesmith158

This program is amazing. You wont regret the decision to spend a year in St Louis. Some of the most progressive EMS systems I’ve seen in my career.
🚨EMS Fellowship Opportunity🚨 @WUSTL_EMS in St. Louis has spots open for their EMS Fellowship THIS YEAR, with start date July 2023. Please help spread the word to interested EM Seniors Ping current fellow & former EMRA PDM VC @bfeiny with any questions

💯 we can and should find a way to initiate cold water immersion in the field. Supervisor/battalion chief carrying a preloaded cooler of ice during times of the year of high incidence could work.
3 takehomes from this episode on exertional heatstroke: 1)Take a rectal temp. Put this in the protocol. 2) It is a time-critical diagnosis to be TREATED ON SCENE. 3) Cooling is the #1 priority. @ReelEmergency @ProdigyEMS @HandtevyMD @ZDunlapmedic @UKROBL1 @gateshil

Do not delay rapid cooling in the heat stroke patient. Any invasive procedures can and should be performed simultaneously. sciencedirect.com/science/ar…

Our EMS Fellows have vehicles to do field response. Dr. Bryan Feinstein responded to a call where the Spanish Lake Fire crew resuscitated a critically ill patient before he arrived on scene. The crew received a Clinical Save Award for their excellent patient care. @bfeiny
