1/8 š§µAs an MD & software developer, I've been reflecting on @ONC_HealthIT's #HTI1 rule, which proposes recording patient preferences on data sharing without required providers to honor them. This can disappoint -- and it misses more pragmatic opportunities for patient control.

If you havent seen this from Sunday's NYT, please read now. The central thesis resonates strongly. It is too easy to conflate MD "burnout" as related to "too much work," whereas it may be more connected to moral distress about health & healthcare in US.

An uncomfortable reminder of why interoperability matters, is inevitably going to get better, and is nowhere close to where it needs to be in the real world application of regulations and standards.

Interoperability canāt get to the outpatient setting fast enough.
Weāre working with an individual whoās getting evicted at the end of the month. We got them pre-approved for a housing waiver but the final application needs a piece of information thatās in their medical record.

I had a similar situation come up years ago while under insured and working on a software tool to help people navigate non-ED options. The experience can only be described as infuriating.
