There has ALREADY been a lot written about NYU @EvidenceOpen @UpToDate Expert AI study but wanted to give my perspective as what counts for an "expert" in human-computer interaction these days. Especially when I see Twitter debates about item response theory. 🤣 A 🧵⬇️
For medical information, general AI frontier models (Google, OpenAI, Anthropic) outperformed specialized @EvidenceOpen and @UpToDate as assessed by 12 US clinicians, randomized and blinded to which model and extensive testing/benchmarks. This was not anticipated. @NatureMedicine nature.com/articles/s41591-0…

Very important study that should energize clinicians and clinical leadership to ask what we want from AI decision-support. What are we maximizing (e.g. accuracy, reproducibility. chain-of-evidence, contextual-specificity etc)? HT @arjunmanrai

I really think the stories about physician compensation are not helping the public perception. Orthos have gripes with reimbursement shifting to PCPs (when orthos are making $600k ) and PCPs are complaining about $330,000 not keeping up with inflation. forbes.com/sites/brucejapsen…

This issue exists in non physician compensation in hospitals, health plans, etc as well. We need to shift the conversation to value for the patient

Other than those repping their home country jerseys, the World Cup vibes in the US are disappointing compared to 1994

I gave a talk last week and afterwards someone summarized it and published a great overview. However it was too great. My points were not that well structured. My guess is that AI wrote it, and realizing that bums me out. Not totally sure why

Maybe I feel like this because, with AI, anyone can put together pretty coherent strategic thoughts Or maybe because it made me realize my talk wasn’t as strategically coherent as I thought 🤣

This is the actual bottleneck. The models are smart enough already. What is missing is the company-specific context locked in senior people heads. Whoever cracks knowledge extraction at the company level unlocks the rest. As you work on this, please consider using GBrain as your OSS retrieval layer x.com/t_blom/status/20608063…

Imagine replacing 90% of your employees with a team of geniuses who have no idea how your company operates. Total chaos. Nothing works. That’s what AI feels like today. The missing piece is extracting all the domain knowledge from people’s heads and providing that as structured context to the models.

the web 2 era subsidy was: “we’ll lose money to acquire users.” the ai era subsidy is likely: “we’ll lose money to acquire data.”
Today, we're launching shift. We're starting by cleaning your apartment in New York City, for free. Here's how it works. Book a shift cleaning. A vetted shift operator comes to your home wearing one of our devices. They clean. They leave. You pay nothing. In exchange, we record the cleaning. Robotics is being built on data about how people do daily tasks, and the value of that recording is what funds the service. Anything personal in it is anonymized before the recording is processed. By now, you have heard about the shift to AI more times than you can count. About the shift toward you, the part where you actually feel it, you have heard almost nothing. Shift is what starts to make it concrete, in specific cities, with specific services. Today, cleaning in New York. Soon, handymen, repairs, and errands across the globe. And this is just one side of shift, with more on the way. Comment “shift” and we’ll send you an early access link.

Lots of people talk about Insurance bots battling health system bots. But now we are talking about patient bots having to battle health system bots? This is insane

I think AI’s greatest promise and potential for good is in healthcare. From personalized medicine to eliminating cancer as a leading cause of death, we’re entering a new era of medicine. We’ll also see an entirely new healthcare software stack emerge to replace systems like Epic that clinicians universally dislike. Excited to see the incredible lineup from @statnews today at #StatBreakthrough. 👏 @mariojoze
