What distinguishes the brain organisation of someone with Parkinson's disease from someone with Lewy body dementia? This Nature Communications study by Zarkali et al. uses diffusion and resting-state fMRI to map structural and functional cortical gradients — and finds they diverge significantly between PD and LBD. What strikes me is the implication: despite sharing alpha-synuclein pathology, the cortical reorganisation patterns suggest fundamentally different network-level vulnerability. LBD shows more pronounced disruption in higher-order association cortices, which aligns with the earlier and more severe cognitive decline we see clinically. The multiparametric MRI approach here — combining structural gradients with cytoarchitectural mapping — offers a framework for understanding why some patients with Lewy body disease develop dementia early while others maintain cognition for years.
