The American Cancer Society has updated its colorectal cancer screening guideline to include four additional tests: • Guardant Health’s Shield blood test • Freenome’s SimpleScreen blood test • Geneoscopy’s ColoSense at-home stool RNA test • Abbott’s Cologuard Plus stool DNA test The key point is that blood-based CRC screening has now formally entered the ACS guideline. This is an important step for improving screening uptake, especially among people who remain unscreened. However, the positioning is cautious. Stool-based tests and visual exams, including colonoscopy, remain the preferred screening options. Blood-based tests are not being placed on equal footing with colonoscopy or stool-based tests, because their sensitivity for advanced adenomas and advanced precancerous lesions is lower. In other words, blood-based screening should be viewed primarily as an alternative for individuals who are unwilling or unable to complete stool-based testing or visual exams — not as a first-choice replacement. The age recommendation also remains important: average-risk adults should start regular CRC screening at age 45, and continue through age 75 if they are in good health and have a life expectancy of more than 10 years. Beyond that, screening decisions should be individualized. The real message: more accessible options are welcome, but test selection still matters.
