Problem with PDAC trials is that high drop outs and early recurrence in both groups makes finding definitive benefit difficult in ITT neoadj v. adj trials. Benefit is reserving surgery for the right subset and delivering sys therapy. Hoping Alliance and PREOPANC prove me wrong.

I feel that resectable panc cancer is a system disease so upfront systemic treatment seems sensible…..it’s just so far I don’t think there has been any good randomised data to support it. Some that go against….

My bias is still neoadjuvant for resectable, with exceptions. Pending results of Alliance and Preopanc-3, I’ll follow the data. Hopefully clearer than mud 🤞

No routine drains for patients after distal pancreatectomy. Selective drains in patients with low fistula risk score (0-2). Always drain FRS >3 and place on extended antibiotics (10d total). CR-POPF <10%.

We have to take care to generalize these results from the PANDORINA and PANDRA trial. In both, the population has a low risk of developing fistula (D-FRS) and the PANDRA trial didn't measure the pancreatic duct. The best polices are still estimating risk.
