Ahead of @ASCO 2026, we asked leading AI models to forecast outcomes for key GU Cancer trials before the data drops. We analyzed 4 major GU cancer trials, generating 42 predictions across survival and response endpoints, along with 8 AI-modeled Kaplan–Meier curves to visualize how outcomes may separate over time. Each forecast includes hazard ratios, statistical significance (p < 0.05: Yes/No), median survival estimates, ORR, and duration of response projections across settings. Highlights: Strong AR pathway intensification signals in prostate cancer TALAPRO-3 (mCRPC) Talazoparib enzalutamide shows one of the strongest consensus efficacy signals across GU trials ▶️ AI consensus rPFS HR: 0.51 ▶️ Median rPFS: ~35.6 vs 19.3 months ▶️ OS improvement trends positive but remains less certain PROTEUS (Localized prostate cancer) Apalutamide ADT demonstrates consistent benefit across metastasis-free and event-free survival endpoints ▶️ AI consensus MFS HR: 0.65 ▶️ AI consensus EFS HR: 0.65 ▶️ pCR: ~19% Renal cancer: modest directional improvement RAMPART (Renal) Adjuvant durvalumab trends toward DFS improvement, though significance remains inconsistent across models ▶️ AI consensus DFS HR: 0.84 ▶️ OS separation appears limited in current projections Ovarian cancer: clearer PFS than OS signal CHIPRO (Ovarian) Chiauranib paclitaxel shows consistent PFS benefit projections ▶️ AI consensus PFS HR: 0.70 ▶️ Median PFS: ~6.6 vs 4.6 months ▶️ OS significance remains uncertain across models ▶️ ORR: ~35% Across trials, the AI consensus points to continued momentum for AR-targeted intensification strategies in prostate cancer, more modest expectations for adjuvant immunotherapy in renal cancer, and clearer PFS than OS differentiation in ovarian cancer. We asked leading AI models to simulate oncologist poll responses before the actual results were announced and compared their predictions to the original poll by @TwoOncDocs ( Dr. @SamArmstrongMD & Dr. @DrKarineTawagi) on Urothelial Carcinoma 👉 x.com/TwoOncDocs/status/2048… Across models, the AI consensus strongly favored bladder-preserving chemoradiation, with: ▶️ TURBT → 5FU/MMC RT (66%) ▶️ TURBT → Gem/cis RT (26%) Clinicians showed a more distributed approach: ▶️ TURBT → 5FU/MMC RT (40.0%) ▶️ TURBT → Gem/cis RT (30.0%) ▶️ EVP → cystectomy (20.0%) Both AI and oncologists aligned on bladder-preserving chemoradiation as the leading strategy, supported by BC2001 data and established radiosensitization approaches. AI placed greater confidence in 5FU/MMC-based regimens, reflecting strong trial evidence and standard-of-care positioning. Oncologists, however, demonstrated more openness to emerging EVP-based strategies, even in a bladder preservation context—highlighting a willingness to incorporate evolving data into real-world decision-making. AI captured the backbone of evidence-based care, but underestimated clinical appetite for newer, less-established approaches.
