RHD affects 54.7M people, kills >370K/year, and disproportionately hits young people in LMICs. Yet there’s been zero progress on disease-modifying treatment. SGLT2i’s anti-inflammatory/anti-fibrotic effects could interrupt valve progression before surgery becomes inevitable.

📢 Our latest editorial explores the "Ivory Tower Paradox": tertiary hospitals in #GlobalSouth delivering world-class care, research, and training while standing on the frontlines of scarcity and health inequity & struggling under the weight of serving millions with finite resources Capability is not the same as capacity Resource scarcity is not the absence of expertise.... It is the gap between overwhelming need and finite capacity The future of global health may be shaped not by those with the most resources, but by those who have mastered excellence despite constraints —and by the patients they refuse to leave behind 🌍⚕️ 🔗 journals.sagepub.com/doi/10.… #GlobalHealth #HealthEquity #GlobalSurgery #AcademicMedicine #PublicHealth #LMICs #MedTwitter

Women structural heart specialists in LMICs drive incredible progress for patients & communities, yet access to leadership development opportunities remains unequal. The Global Leadership Initiative was created to help change that. Apply now: womenasone.org/for-physician… #GLI2026

Unless Elon is keeping fat people from ultra processed foods, this is the wrong link. You probably meant paper published in the The Lancet which is an unvalidated study. This study author acknowledges that: "Despite the US Agency for International Development (USAID) being the world's leading donor for humanitarian and development aid, there is scarce evidence in the literature assessing its impact on global health. Few evaluations have attempted to estimate the effects of USAID funding on maternal and child mortality in selected low-income and middle-income countries (LMICs), and some reports have offered only approximate estimates for specific diseases." And that ", this study is the first comprehensive analysis to assess the impact of total USAID funding" So we have scarce evidence USAID impacted anything for twenty years, yet now this unvalidated study suggests through statistical analysis its absence has caused mass deaths.

3/3 This is global. For the West: value-based care. For India/LMICs: access and affordability. For patients everywhere: value is not only PFS, it is life beyond continuous treatment. The next disruptive myeloma trial may be the one that defines when “enough” is truly enough. Finite therapy in myeloma will need courage BUT more importantly, it will need evidence. @VincentRK @RahulBanerjeeMD @DrOlaLandgren @AjaiChari @BrunoPaiva_UNAV @End_myeloma @SagarLonialMD

Attitudinal changes will require some form of interventions, particularly from govts in LMICs. If we cannot make provisions for people to dump waste, it will be difficult to ask them not to throw them indiscriminately into the environment. We need to make the provisions first.
