In April, Kawasaki Aortic Center welcomed nine international doctors and medical professionals at the same time, including short-term fellows and visitors from Taiwan, Thailand, the Philippines, and Ethiopia. Sharing our daily practice, surgical strategies, perioperative care, and philosophy of complex aortic surgery with such motivated colleagues is a great source of inspiration for us. Recently, we were able to support an international patient with a highly complex aortic condition through open surgical treatment, postoperative recovery, and safe return home. At Kawasaki Aortic Center, we evaluate complex aortic diseases, including redo cases, extensive thoracoabdominal aneurysms, post-TEVAR complications, and infected grafts. Even when treatment options seem limited, open surgical reconstruction may still be possible in carefully selected patients. For young patients with heritable conditions like Loeys-Dietz Syndrome 🧬 (LDS), choosing a durable approach is critical. This Case Report in the #EJVESForum details the successful open surgical management of a large IIAA in a patient with LDS type I. 💡 KEY FINDINGS: A 41-year-old woman presented with lower abdominal pain and a 4.8 cm right IIAA. Genetic testing confirmed a TGFBR1 mutation consistent with LDS type I. · Management Strategy: Open surgical repair was chosen to ensure a durable, anatomical reconstruction in a young patient with heritable disease. · Surgical Technique: A right retroperitoneal approach was used to access the aneurysm. · Handling Fragile Tissue: To address the tissue fragility of LDS, surgeons used an everted arterial wall and felt reinforcement for the anastomosis. · Outcome: Six-month imaging confirmed a widely patent bypass graft and successful preservation of pelvic perfusion. · Safety: The patient recovered without complications and was discharged home on post-operative day 5. 👉 TAKEAWAY: Durability in Heritable Aortic Disease While endovascular options are increasingly common, open surgical repair remains a highly effective solution for IIAAs in young patients with heritable vascular disease.
