📘PFAで後壁を治療するとPersistent AFの再発は減るのか？
PersistentAFにおける治療戦略として注目される「左房後壁（LAPW）追加アブレーション」。
本研究はPFA時のLAPW追加が再発予防に有効かを、MANIFEST-PFレジストリ547例を解析して検証。
👨⚕️結論：PVI単独 vs PVI LAPW、再発率は同等（1年後）
1年後の再発回避率：
PVI単独 73.1% vs PVI LAPW 66.4%（P=0.68）
Propensityスコアで補正しても有意差なし
合併症率も両群で差はなし（LAPW群：2.2% vs PVI単独：1.4%）
🛠️Tips
「LAPWも焼いておきたい」という気持ちはあるが、PFAでは後壁のDurable isolationでも成績改善に結びつかず。
追加通電＝長時間化にもつながる。
PFAでは“最小限で最大効率”を意識した戦略がカギか。

Yes, it has a mild LA enlargement.
But, indeed, persistentAF was documented in COVID pandemic.
DCCV after transeptal puncture before map acquisition was unsuccessful.
Voltage map remodeling had a correlation with time on AF.
Pt D/C home in SR.
