Wow... thanks for all the new follows, everyone! I'm working today (1 pharmacist basically verifies most of the orders for the entire 168 bed hospital, another does clinical dosing for heparin, aminoglycosides, and warfarin, formulates TPNs, and monitors our renal patients, and the 3rd checks all outgoing IV medications, patient's own meds, and kits for the floor, then helps wherever she can!), but I'll try to follow you all back after my shift ends!

😴🫁 New AASM guideline updates treatment of central sleep apnea in adults ◆ A new American Academy of Sleep Medicine clinical practice guideline in J Clin Sleep Med updates management of central sleep apnea (CSA) and stresses individualized, patient-centered care—prioritizing quality of life and functional outcomes, not just AHI suppression. ◆ All nine recommendations are conditional (GRADE), covering six options tailored to etiology: CPAP, bilevel PAP with backup rate, adaptive servo-ventilation (ASV), low-flow oxygen, oral acetazolamide, and transvenous phrenic nerve stimulation (TPNS). Choice of therapy should be anchored in the underlying cause (e.g., HF, OSA overlap, opioids). ◆ ASV receives a conditional recommendation across multiple CSA etiologies, but in HFrEF it should be restricted to experienced centers with close follow-up. TPNS is newly recommended for primary CSA and HF-related CSA, yet its invasiveness, cost, and limited access mean PAP and medical options remain first-line for many patients. A new spark for steadier hands? An AI-Driven nerve stimulation eased essential tremor in a randomized study. Transcutaneous peripheral nerve stimulation (TPNS) is a technique that uses mild electrical impulses delivered through the skin to activate nerves in the arm and influence brain circuits that may control tremor. Ondo and colleagues describe, in a new paper that just dropped in JAMA Neurology, that a randomized clinical trial testing of an artificial intelligence driven wearable device stimulating the radial, median and ulnar nerves could reduce tremor severity in folks living w/ essential tremor. Key Points: - The study enrolled 125 adults and randomized them to receive active TPNS or sham stimulation for 90 days across 12 neurology centers in the USA and China. - Participants using TPNS improved their modified Activities of Daily Living scores by nearly 7 points vs 2.7 points in the sham group. - There were meaningful gains in eating, drinking, writing and dressing. - The most common side effect was mild skin irritation in about one-third of users. My take: It is nice to see an AI driven device based approach to treat essential tremor in those w/ mild to moderate symptoms. DBS and FUS still reign supreme when a next step is required, but this is pretty cool. 1- Essential tremor is one of the most common movement disorders and frequently limits hand control for daily tasks like drinking and writing. 2- Many folks do not respond well to medicines for tremor, creating a need for new and safe options. 3- This AI-guided nerve stimulation device improved tremor related daily function over three months without requiring brain surgery. 4- The results mark the first large blinded multi-center 'nonsurgical' trial in essential tremor in more than two decades to meet its main goal. 5- Continued followup will be the key to see if benefits persist long-term and whether wearable stimulation will become a routine part of tremor care. jamanetwork.com/journals/jam… @ParkinsonDotOrg @essentialtremor @SfNtweets @movedisorder #tremor @DBSThinkTank
