You hear frustration in their voices. An engine crew and ambulance crew – handling two separate critical EMS calls – can't get anyone at DC911/@OUC_DC to answer the radio. They called 10 times over five minutes last night. Help was delayed. It's at least the 7th similar failure in as many months. Here on election day, will any of the candidates finally make fixing DC911 a priority? These weren't the only major issues at OUC last night. HERE’S ANOTHER ONE: I told you yesterday that there were definitely more DC911/@OUC_DC mistakes over the last six months that I didn’t know about, beyond the 37 I posted Tuesday night. Well, number 38 came to my attention yesterday. It’s another sad one. A woman died. It occurred last Friday. That’s the same day that a US Park Police officer had to drive an apparent stroke victim to the hospital after OUC dispatched EMS to two wrong locations. The new incident was initially dispatched as an unconscious person to 1221 4th Street NW. Six minutes into the call, a dispatcher said he was told the first address was wrong and that the correct location was about a mile away at 1221 M Street NW. What wasn’t relayed, that had been entered into the dispatch computer, was that the unconscious person was now reported to be in cardiac arrest. Of course, that’s important information that should have been shared on the radio with the responding units. The dispatchers should have also upgraded the call. That would have added an EMS supervisor. The EMS supervisor wasn’t dispatched until 16 minutes into the call, after Engine 6 reported they were finally with the patient who was in cardiac arrest. I can’t definitively say at this point if the address mix-up was the fault of the 911 caller or OUC. But failing to relay crucial information and not promptly upgrading a call are common mistakes at OUC. It’s exactly what happened during the December 2nd cardiac arrest, which is the very first incident on the list I’ve compiled. LISTEN: Why is this tolerated? Yesterday, once again, DC911/@OUC_DC dispatchers failed to answer the radio. They ignored five calls in almost three minutes on the EMS channel for Southeast and Northeast. At least this one didn't delay help to a patient in cardiac arrest as the last one did. Director Heather McGaffin has allowed this to occur at least 23 times since she took over three years ago. And these are just the ones I've learned about. What does it say about McGaffin's leadership that she can't solve this problem? What does it say about @MayorBowser & @SafeDC that they don't have a 911 director who has the skills to fix this (remember, Bowser fired the director who effectively addressed this issue)? What does it say about @CMBrookePinto that she hasn't demanded transparency and accountability on this issue in oversight hearings? McGaffin submitted testimony in February that when these incidents occur, they're listed on the Performance Dashboard (see below). Not one of the nine this year has made it to the Dashboard, and only two of the eight last year were reported. Yet, CM Pinto is okay with that, telling McGaffin in June that my reporting on this can be ignored because I don't know the phone number of the 911 callers. Isn't that an excellent way for an agency to keep accurate performance stats? This abandoning of emergency radio channels is not a chronic problem at other 911 centers, but it has been a serious OUC issue for almost six years. What else could OUC's staff be doing that they're ignoring the radios so frequently? I'll shed some light on that very soon. As usual, I'll have the receipts. Imagine it's 8:00 p.m. on August 2nd. DC911/@OUC_DC Director Heather McGaffin steps up to a bank of microphones with bright lights and cameras aimed at her. McGaffin says the following: "This was a bad day for OUC. A series of events this afternoon resulted in DC911 not providing the level of service it should. It's my job to fix what went wrong and I will. "This began with a dispatch computer crash. It was caused by an improperly handled update by a contractor. As of this afternoon, the contractor no longer works for the city. "This shouldn’t have occurred, but 911 centers must be able to operate effectively when computers crash. Without the computer, our staff answered many 911 calls and dispatched help. Our problem was we didn’t operate proficiently and consistently throughout the two-hour computer outage. Mistakes were made. "We lost track of where some fire and EMS units were located, and which ones were on calls. We also failed in some cases to verify that fire and EMS received a dispatch and were actually responding. This occurred several times, including during a cardiac arrest call for a baby boy on Connecticut Avenue NW. We know that delayed getting Advanced Life Support units or paramedics to the baby. We regret this occurred. We are heartbroken this child died, and we send our condolences to his family. "There was another issue connected to the Connecticut Avenue call that’s directly related to our ongoing staffing shortage. It appears the little boy's family called 911 about 10 minutes before we learned of the emergency. They hung up when we didn't answer promptly. Again, that problem was not related to the computer outage. Longer waits are unfortunately a fact of life when you don't have enough call-takers. If you call 911 with an emergency please stay on the line when you get the recording. I know it's frustrating, but if you hang up, when you call back your emergency will go to the end of the line. Stay on the call. "We continue to work on our staffing issues. With fewer people it has been difficult to schedule training and conduct drills on things like handling a computer crash. Still, we will find a way to do that training on a regular basis and make sure we are prepared. We must do better and we will do better. I’m ready to answer your questions." While Heather McGaffin coming forward and saying all of this two week ago today is pure fantasy, the content of the message is not. It’s generally what occurred. It honestly describes the events of August 2nd. Instead of being open and direct with the people they serve, Mayor Muriel Bowser’s administration shared little bits and pieces of information throughout the last two weeks. No one appeared on camera and none of the information they provided in any way addresses how the call for the baby in cardiac arrest was so badly handled. Instead of getting the bad news out quickly and getting it behind you, these tactics have brought two solid weeks of DC911 news stories. How is this better for DC911’s image? How does this increase trust in 911? How is this better for anyone, including the family of the dead child? As I predicted two weeks ago, the Bowser administration would follow a very flawed game plan of hiding the truth and refusing to answer what happened inside DC911. It’s the same plan they followed with the District Dogs flood a year ago and with many other 911 failures. Heather McGaffin isn’t providing the transparency and accountability she promised under oath before her early 2023 confirmation. She is also failing in another big way. A message to the public about not hanging up when you get the 911 recording is crucial. No one in DC government has shared that message during this entire ordeal. Because McGaffin has avoided reporters for the past year she has consistently missed the opportunity to regularly get that message in front of the public. Imagine being so scared of answering tough questions you abandon the opportunity to help the public, and keep them from making potentially tragic mistakes. What we are witnessing is the opposite of good leadership. DC911 Crisis Day 14: Another excellent article by @jennygathright on the long list of problems facing DC911/@OUC_DC. One of many news stories this week, including a bit of attention from outside of DC (we need more of that - hello @nytimes). Through it all, not a single word uttered to reporters from OUC Director Heather McGaffin. No one has answered why DC911 was unprepared to handle the August 2 computer crash. Why dispatchers made multiple mistakes trying to see paramedics to a dying baby. With @MayorBowser's administration's strange reputation management game plan of keeping the bad news going day after day by letting the information slowly drip out, it's no wonder the reporters are staying on the story. It's also no wonder Bowser and company are searching for leaks, with MPD asking @tednbc4 for his sources. I strongly encourage the reporters to stay with this story and keep digging. Maybe some reporter will get lucky and come up with the CD1 forms that the Bowser administration is illegally withholding, claiming to those filing FOIAs they can't be found. Those forms provide a much more extensive and detailed list of OUC's many errors than I've gathered over the last five years. Likely, they're the only true and honest accounting within the DC government of OUC's daily mistakes. Those forms will show you the patterns, the repeat mistakes due to the major systemic failings at OUC. And don't forget the other major bit of data that Director McGaffin has testified she WILL NOT release - how long it takes to actually dispatch a call. McGaffin claims she can't share this crucial data concerning the health of any 911 center because @DCPoliceDept won't let her (imagine that). The reason that it's withheld, despite a law that requires its release, is because the Bowser administration knows the public will be outraged when they learn just how long it takes to dispatch even the higher priority police calls. NEW: Great report this evening by @jennygathright explaining in detail some of the mistakes made at DC911/@OUC_DC surrounding the death of a five-month old child. It verifies what I have been saying that this goes much deeper than a contractor making a mistake on an update. This touches on all of the things that ail 911 in the District of Columbia. It has been clear DC911 has been broken for a long time. It shouldn't take the death of now three babies since 2022 or ten dogs last year to get the attention of city leaders to fix this. They have known these problems for years. The DC auditor verified many of the things STATter911 has been reporting. Last June OUC's director even verified they don't have enough staff to promptly answer 911 calls. Yet the status quo continues. And people who are dying can't get help when it's needed. A suggestion for @CMBrookePinto: You are in charge of oversight. Show you care. Stop issuing weak statements and actually talk with reporters explaining how you will address these known problems on your watch. "Meltdown" is not hyperbole. It's exactly what happened Friday at DC911/@OUC_DC. Dispatchers created a chaotic situation for hours, unable to keep track of which fire & EMS units were available and which were on calls. A victim in all of this was a 5-month old boy in cardiac arrest. Firefighter/EMTs waited at least 20-minutes for paramedics because of OUC's confusion. NEW: There were good intentions by the 911 caller and those working at DC911/@OUC_DC after someone passed out during this afternoon's White House press briefing. But its handling has me again questioning procedures, supervision, and training at DC911. Is it wrong to think that when the 911 center in the nation's capital gets an outsider calling about an emergency at 1600 Pennsylvania Avenue NW there's enough communications sophistication that someone in OUC is immediately talking with @USSecretServic1 about an emergency response to the White House? Instead, it was treated as any other call. It was left to an incredulous @dcfireems EMS supervisor (listen below 🔽🔽) to read the notes for the call to realize the response was initiated by an apparently concerned citizen watching the press briefing on TV when the person passed out. I am not claiming EMS shouldn't have been dispatched. It's usually better to get emergency crews on the road. Dispatchers should have immediately explained the situation to the responding units and told them they were on the phone with US Secret Service to confirm EMS is actually needed. But this requires more than the DC911 center can currently muster. It requires call-takers who alert supervisors about unusual 911 calls. It requires dispatchers and supervisors paying attention to the calls and reading the notes. It requires enough well-trained staff to allow the 911 center to operate effectively. Sadly, none of these are things are regular features at DC's 911 center. They also canceled an engine and ladder truck going to the right address where a mother and her baby were trapped. NEW: It happened again early this morning. For the third time in 10 days DC911/@OUC_DC botched the dispatch for a building fire. This time a mother and baby were hunkered down in the bathroom of a smoke-filled apartment waiting to be rescued. They waited much longer than they should have because @dcfireems was sent to the wrong location. These things keep happening because DC911 is in crisis and being neglected by @MayorBowser's administration.
