Phase 2 multicenter trial of chemoimmunotherapy for patients with neuroendocrine or aggressive variant metastatic prostate cancer (CHAMP).

The phase II CHAMP trial combining cabazitaxel carboplatin with dual checkpoint blockade (ipilimumab/nivolumab) met its primary endpoint in metastatic NEPC/AVPC, achieving a remarkable 78% 6-month rPFS rate and a median rPFS of 12 months.
This heavily pretreated cohort included 75% of patients with visceral metastases. Importantly, toxicity was manageable with no treatment-related deaths, and 2 patients remain disease-free off therapy beyond 30 months.
These data provide a strong rationale for randomized trials of chemoimmunotherapy in NEPC/AVPC.

CHAMP met its primary endpoint: 6-month PFS 74% vs 55% historical control, with 23% 2-year survival in a population where platinum-taxane alone rarely gets patients past a year.
The randomization question is the right question. Single-arm with historical comparator is often the only feasible design when you’re enrolling NEPC/AVPC, but it makes regulatory translation hard. A randomized phase 2 would require network coordination and sustained sponsor commitment to a rare, rapidly progressing population. Worth pushing for.
This is the most actionable data we have in this space. Well done to the Duke team.

#CHAMP is an important trial led and presented by @AarmstrongDuke from our @DukeGUCancer team evaluating chemoIO with cabazitaxel/carboplatin and ipilimumab/nivolumab in patients with aggressive variant or neuroendocrine metastatic #ProstateCancer. Maintenance IO continued after up to 10 chemo cycles.
Met primary endpoint of 6 mo PFS 74% vs 55% historic carbo/cabazi with expected but manageable toxicity. 23% survival at 2 years is much better than historical data. Two patients alive and off therapy at 30 months!
Aggressive treatment for aggressive disease, but shows chemoIO should be considered as SOC for these patients with historical poor outcomes.

Cabazitaxel carboplatin ipilimumab/nivolumab — “CHAMP” regimen

This is a clinically important space.
Aggressive-variant prostate cancer and small-cell/neuroendocrine prostate cancer remain among the hardest prostate cancer phenotypes to treat.
They often behave differently from typical AR-driven mCRPC:
🔹 low PSA biology
🔹 visceral metastases
🔹 TP53/RB1/PTEN loss
🔹 platinum sensitivity, but short responses
🔹 very poor prognosis
The question:
➡️ Can adding dual checkpoint blockade to platinum-taxane chemotherapy improve outcomes in AVPC/NEPC?
🔵 Study population
N = 40 safety population
This was a very high-risk cohort:
• AVPC: 70%
• NEPC: 30%
• visceral metastases: 73%
• liver metastases: 58%
• prior ARPI: 93%
• prior chemotherapy: 78%
• TP53/RB1/PTEN alteration/loss common
So this is exactly the population where new strategies are urgently needed.
🔵 Primary endpoint
6-month immune-modified rPFS:
➡️ 74%
vs historic control 55%
➡️ p = 0.013
Median immune-modified rPFS:
➡️ 12 months
That is a meaningful signal in this aggressive biology.
🔵 Secondary endpoints
RECIST rPFS:
➡️ median 6.1 months
Overall survival:
➡️ median 12 months
Objective response rate:
➡️ 37%
Median duration of response:
➡️ 9.3 months
2-year survival:
➡️ 23%
🟠 Toxicity
Chemo-immunotherapy is not easy:
➡️ Grade 3 related toxicity: 16%
➡️ Grade 4 related toxicity: 3%
But as the presenter emphasized:
the cancer is toxic too.
In AVPC/NEPC, the treatment-risk discussion must be framed against the biology of the disease.
🔵 My take
This is not a practice-changing phase 3 result.
But it is an important signal in a population with very limited options.
The key message:
Dual PD-1/CTLA-4 blockade with cabazitaxel/carboplatin appears feasible and shows encouraging activity in AVPC/NEPC.
Next questions:
➡️ Which patients truly benefit?
➡️ Is NEPC different from AVPC?
➡️ Can TP53/RB1/PTEN or other biomarkers refine selection?
➡️ How should this be sequenced with platinum, PARPi, ADCs, and radioligands?
➡️ Can efficacy improve without excessive toxicity?
For aggressive prostate cancer, biology matters — and treatment strategies need to match the biology.
