This one-pager covers:
• Initial workup & essential biomarkers (HER2, PD-L1 CPS, MSI/dMMR, CLDN18.2, NGS)
• Perioperative FLOT now preferred (superior OS vs CROSS per ESOPEC)
• CROSS as alternative for bulky/borderline disease
• Adjuvant nivolumab (CheckMate-577) after CROSS if residual disease
• Biomarker-driven therapy in metastatic & unresectable settings
• High-yield clinical pearls

🚨 New standard of care in resected EGFR-mutated NSCLC!
Adjuvant Osimertinib for 3 years after complete resection of stage IB–IIIA EGFR (Ex19del or L858R) non-metastatic lung cancer is now Category 1 recommended by NCCN, ESMO & ASCO.
From the practice-changing ADAURA trial:
• DFS at 24 months: 89% vs 53%
• Risk of recurrence or death reduced by ~80% (HR 0.20)
Beautifully summarized by @DrRupamOncology — perfect quick-reference for oncologists, thoracic surgeons & residents.
Early EGFR testing adjuvant TKI = game changer for never-smokers & Asian patients too.

Clinical question: For cT2N0 esophageal cancer, should we proceed with primary surgery or start with neoadjuvant therapy?
Our collective effort through the @ISDE_net to address this clinical dilemma is now published.
Key takeaways:
• Neoadjuvant therapy → surgery is suggested for most patients
• Clinical staging is imperfect, with nodal upstaging reported in up to ~48%
• Primary surgery may still be appropriate for carefully selected low-risk tumors
Overall, the guidance aligns with the @NCCN guidelines many cT2 tumors.
The challenge now is better risk stratification and staging to identify who truly benefits from neoadjuvant therapy.

Precision oncology in stage IV NSCLC is no longer optional. It is the backbone of care. 🧬🫁
One diagnosis. Multiple molecular roads. Very different treatments.
What matters first? Broad NGS upfront. Always.
🔹 EGFR alterations
Exon 19 del / L858R: osimertinib ± chemo or amivantamab lazertinib
Exon 20 insertions: chemo amivantamab
Resistance pathways now include MET, T790M, ADCs, bispecifics
🔹 ALK, ROS1, RET, METex14, BRAF V600E, NTRK
Highly effective targeted TKIs upfront
Sequencing matters more than ever after progression
🔹 KRAS G12C, HER2 exon 20, NRG1
Targeted options mainly in later lines
ADCs and next-gen TKIs changing outcomes
Key takeaway 👇
Stage IV NSCLC is not one disease.
Treat the driver, not just the histology.
📌 Broad molecular testing at diagnosis = survival-defining decision.

🚨BREAKING: NCCN SCLC Guidelines now include Lambert-Eaton Myasthenic Syndrome (LEMS) 🫁
📌 VGCC antibody testing neuro consult to diagnose LEMS
✅ Amifampridine may be considered for supportive care

I will start to occasionally share on X breakthroughs at Natera (NTRA) and my other companies that I hope you will find interesting and inspiring. @NateraGenetics has generated over 200 peer-reviewed papers and some transformations in healthcare. The latest is in #CancerCare: Simultaneous papers in Nature Medicine and Annals of Oncology released data from the Circulate Japan Trial. Results show a 10x overall survival improvement for Signatera-positive vs. negative colorectal cancer (CRC) patients and that we can at last decipher who should and shouldn’t receive chemotherapy after tumor removal. This technology can potentially save the US healthcare system more than any single drug repricing in the Inflation Reduction Act and save many patients the side-effect and emotional burden of unnecessary chemo. This already led to updates in the Japanese Society for Cancer of the Colon and Rectum (JSCCR) guidelines, marking a significant shift in global care. Congrats and thanks to the superb team at Natera; we’ve surpassed 85 publications on Signatera alone. Follow me here for updates.

#cancerrehabilitation should be a standard in cancer care.
#cancerguidelines recommend rehab for patients during treatment to help them keep up w/their daily activities. BUT current medical practice often falls short of including rehab in cancer care

REHABILITATION: Cancer treatment is all about preserving the life and the quality of life of the patient, but often that is not the case when it comes to rehabilitation.
