Agree. We have made this bandwagon jumping mistake so many times in nephrology (high BUN hypothesis, non Ca binders, access monitoring,carnitine, vitamin D, high hgb targets,etc) and ignored the physiology. I would probably start with nutrition. Was the diet deficient in FA?
