Subject: As per special permissions for critical bipolar meds & confirmation of back paid rent Good day, I certainly hope whomever finds themselves reading this that you're havin' an excellent one! I was just able to find the gas money needed to attend yesterdays dr appt (travel reimbursement for last dr. appt two weeks ago is finally in today so I can pay back yesterdays gas money thank you) however I'm afraid the special permissions forms you sent me that I then forwarded to my dr office are possibly the wrong forms or possibly missing a page as there was no where for my dr to sign. We filled them out as best as possible & the clinic themselves faxed them in yesterday. I afraid I am unable to attach all 4 screenshots of said filled out paperwork as well as a screenshot of the $8447.00 TD interact e-transfer I just sent to my landlord to show I paid my rent with the $8447.00 y'all just sent me to do so so I am attaching 1 screenshot to 4 emails. Each marked 1 0f 4 , 2 of 4, 3 of 4, 4 of 4. Thank you so much sincerely for all that you do. Im afraid I was too overwhelmed yesterday & was unable to properly advocate for myself. I sincerely need my meds I think. I was so overwhelmed dealing with everything else I completely forgot to ask for the special dietary letter I need for the special food allowance y'all told me to get to cover the $200. y'all are clawing back for the Disability Tax Credit I am not receiving. So in the meanwhile I guess I'll just keep forgaging for food in the woods to make things meet & the next time I get into town I will sincerely once again try to advocate for myself. I would love it if you could direct me to any program that could help me Advocate for myself because I am not rightly okay here.
