航空機は高度約10,000mを時速900kmで飛行する。客室内の気圧は地上より低い0.7-0.8気圧で、標高2,000-2,500mの山(富士山5合目)での環境に等しい。
健康な乗客では、この客室環境により動脈血酸素飽和度が地上の97%から機内では約90-93%に低下する。動脈血酸素分圧（PaO₂）は地上の95 mmHgから約60-65 mmHgに低下する（Lancet. 2009;373:2067-77）。
SpO2が90%の患者は、2,500mではSpO2が84％に下降する。
SpO2 が＞95％なら問題ないが、SpO2 92−95％では機内での低酸素に耐えられるかどうかを調べる高所シミュレーションテスト(HAST：hypoxia-altitude simulationtest)が必要である。
SpO2＜92%なら検査を行うことなく酸素を処方し、機内で2-3L/分で使用するように指導する必要がある。

Pioneering has often been romanticized – where you get to introduce new language, edgy concepts, and innovation while keeping your comforts. But the reality is that true pioneering costs you everything. It means stepping into the unknown, sacrificing comfort, and often feeling like you don’t fit in the existing systems. It’s a journey of building in secret, where the true cost, struggle, and fight for vision aren’t always visible to others.
At the recent @XRHACK in #stockholm, I had the incredible experience of meeting the community again. I was deeply moved to hear that the work I did with @unity and @Varjo, Volvo Cars continues to inspire, being used as case studies in universities and big companies worldwide. With the Volvo Cars #mixedreality demo in Varjo XR1, and with the Unity SimulationTest Track template that has been used by thousands of people, studios, and schools.
Students and industry leaders shared how my talks, lectures, and posts ignited their passion to enter this field. Hearing that impact, years later, was both humbling and affirming.
It reminded me that the unseen struggles and sacrifices we make as pioneers do have a lasting impact. To all the dreamers and innovators, keep pushing forward. You never know how far the ripples of your work will go. #XR #Innovation #Pioneering #Unity #Varjo #FutureOfTech
