Adequate management of periodontal infections in patients with Type 2 Diabetes Mellitus is as effective as starting a new oral agent. HbA1c reductions of ~ 0.7%. Annual oral check ups are as important as annual fundus exams. #orocrinolgy #endotwitter jpma.org.pk/article-details/…

How long do you think HDFC Bank will take to transfer the money from your account to your nominee's after you die? Public sector banks get a bad rap, but it's been more than a year and we have not been able to get funds transferred from my now deceased husband's account in the country's largest private bank. The nominee is our adult daughter, there are no conflicting claims, the paperwork is clear, and yet for more than a year now HDFC has been giving us the run around. Get this paper, get that paper. Each time my daughter comes home for holidays, we have to take 2 days out to go and wait at the bank branch. Today, more than a year after the first application, they said a) we can't find the paperwork including the death certificate you submitted because the employee you submitted to is not here b) you have to get a stamp paper and notarise it for the auto debits that hit his account between the date of his death and the date the account was frozen (on the basis of the paperwork in point a, now non-traceable) c) you have to get HDFC Standard life to sign off because there is an auto debit for an investment from his account. (ME: they are the payee, he is the payer, why does the payee need to approve stopping a standing instruction? Bank: Hamare main aise hi hai). If you wait for 30 minutes, we'll get you the form. d) after 30 minutes. Sorry, we can't get the form, you have to go to their office and do it. By now it is 2.45 PM, HDFC Standard life office is at least 35 minutes away. They are open only till 3.30 PM. We are not sure we'll make it there on time. I call their number to ask if the branch is open tomorrow. Person: Sorry, I can't give you the information today, you will have to call and check tomorrow. (To repeat: HDFC Standard Life's call centre cannot tell you if a branch is working or not the following day) In all, we have spent many hours over many months trying to get this money in a case the bank officials themselves say "should not be complicated". What is worse is for my 20-year-old daughter to relive the trauma of losing her father each time we have to go and explain the case. Just today, we had to speak to three different people and start the story from the beginning. This is not just the bank's inefficiency, it is its utter callousness. (My husband also had money in Indusind Bank. It took one visit and two weeks for that to be transferred). The other, more important point, is that I have a job, we have the money to continue to pay for our needs without having to immediately access these funds. But I am certain that is not the case for many others. What about them? How can this be the system of a bank whose customers are dependent on it for giving them access to their own money?! It beggars belief. @HDFC_Bank @HDFCBank_Cares

Case shared in CME INDIA GROUP by Dr Subramanian Senthilkumaran,Manian Medical Centre, Erode, India First Reported Case of Hirata’s Disease Following Common Krait (Bungarus caeruleus) Envenomation in South India @drsthanus @DrAkhilX Clinical Case Note: Snakebite-Induced Hirata’s Disease (Insulin Autoimmune Syndrome) Patient Demographics: Age/Sex: 54-year-old female Occupation: Agricultural worker Location: Rural Tamil Nadu, India Past Medical History: No known comorbidities Clinical Timeline & Presentation: Day 0 (Snakebite): Bitten on the left foot by a common krait (confirmed by herpetologist). Developed neurotoxic symptoms: ptosis, hypersalivation, frothing. Treated at a primary care centre with 20 vials (200 mL) of polyvalent antivenom. Discharged after 4 days with full recovery from neurotoxic symptoms. Day 10 (Post-Bite): Presented with altered sensorium and severe hypoglycemia (blood glucose <40 mg/dL). Initial treatment with IV dextrose and corticosteroids provided transient relief. Labs: Extremely high serum insulin, positive insulin autoantibodies. Diagnosis: Insulin Autoimmune Syndrome (Hirata’s Disease). Treatment Administered: Initial therapy: IV glucose, corticosteroids. Definitive therapy: Plasmapheresis, leading to rapid clinical stabilization. Outcome: Full neurological and metabolic recovery without sequelae. Discussion & Clinical Relevance: Novelty: First reported case of snakebite-induced Hirata’s disease. Mechanism: Likely immune dysregulation post-envenomation, triggering insulin autoantibody formation. IAS Pathophysiology: Initial sequestration of endogenous insulin → transient hyperglycemia Subsequent uncontrolled release → postprandial or fasting hypoglycemia Differential Diagnosis Considered: Insulinoma, factitious hypoglycemia, reactive hypoglycemia Confirmed by: High insulin levels, positive insulin autoantibodies, absence of exogenous insulin use. Clinical Message: Be vigilant for autoimmune hypoglycemia like IAS after systemic envenomation, especially with unexplained hypoglycemia post-discharge. Early recognition and use of plasmapheresis can prevent complications and hasten recovery. Training rural clinicians and emergency providers to consider rare post-envenomation sequelae like Hirata’s disease is critical. Citation: Anbu M, Suraweera TL, Janardhanan J, et al. Pleased to attend #ESICON2024 the 53rd annual conference of the ESI the Endocrine Society of India (ESI) @IndiaESI in the coastal city of Visakhapatnam. I am told that ESI is the largest organization of Endocrinology in India with over 1500 qualified endocrinologists as its members. Endocrinology plays a crucial role in diagnosing, managing, & preventing conditions like diabetes & thyroid disorders, among others which are among common lifestyle diseases. Diabetes, both Type 1 & Type 2, is one of the most prevalent chronic diseases linked to lifestyle factors such as diet, physical activity, & stress. Thyroid disorders, such as hypothyroidism & hyperthyroidism, are also common endocrine issues that can be influenced by lifestyle choices, including diet & stress. Endocrinologists use tests like thyroid function panels to identify imbalances in hormone levels, to identify & manage these disorders. Otherwise, they can lead to fatigue, weight changes, & mood disturbances, which significantly impact the quality of life. Endocrinologists are integral to detecting & treating these disorders early, thereby reducing the risk of severe complications. By understanding the link between hormones & lifestyle factors, endocrinology offers a specialized approach to tackling modern diseases, improving health outcomes, & promoting preventive care. —-

There is nothing called multivitamin deficiency. Unless there is deficiency of specific vitamins, Eg. Vit D or B12 , giving multivitamins may not be beneficial and may even do harm. And Multivitamins usually do not provide enough of the deficient vitamin. ⁦@drbabukv

This is long, but worth your time. Please read and share widely. I just came to know about this. A well meaning Endocrinologist and Diabetes specialist @PremNarayanan9 has been sued and dragged to Court by an Ayurvedic company (IndusViva) that makes a quack product called iCoffee for control of diabetes and weight reduction. The defamation suit is for $60,000 (50,00,000 INR). The good doctor had made a one-minute reel on the product, discussing the factual lack of efficacy and safety studies of iCoffee, in good quality human trials. He was sent a legal notice to take down the video, which he did. The video (with English subs) is linked in the next post (tweet). But thereafter, a defamation suit was filed at the Bangalore Civil Court against him, by the company, demanding payment for "monetary loss." In the defamation suit (which I have read through), the manufacturer (IndusViva) mentions that they are a multi-level marketing company and their product is "well tested." The first part of the statement is true, but the second part, not so. iCoffee contains: Salcital (extract from Salacia reticulata herb) Premium Coorg Coffee (in decaffeinated form) Velvet Bean (Mucuna pruriens) extract and an iCoffee Creamer which has 22% Salcitol The price for 50 sachets is 3000INR ($36) This is very expensive for an untested product advertised through extraordinary claims and sold through a pyramid scheme. Extract of Salacia is not standardized in the product. The extract dose is not mentioned. Salacia extract is supposedly beneficial at minimum 1000mg where it acts on glucose absorption in intestines, which still remains to be proven in large quality Phase II and Phase III trials. How much of Salacia does this product contain? No idea. The effectiveness of Salacia in diabetes is questionable since the quality of evidence is very poor and only based on small human studies. examine.com/supplements/sala… There is literally ZERO studies peer-reviewed and published on iCoffee in medical literature. ZERO. pubmed.ncbi.nlm.nih.gov/?ter… The only study that the manufacturer IndusViva cites showcasing "effectivness" of its quack coffee product, in the defamation suit, is actually a substandard study published in a garbage, third-rate, predatory journal called Indian Journal of Medical and Allied Research, the website of which clearly says they will accept papers "within 24 hours." Clownery from IndusViva company at its peak. And I could not even find that study. ijmar.in/ iCoffee also contains Mucuna puriens or Velvet Bean extract which is a high source of L-Dopamine which carries the risk of the side-effects of L-DOPA, which include several gastrointestinal and neurological symptoms. Like other herbs, it always carries a risk of negative interactions with various medications - meaning it can interact with your real diabetes medications. examine.com/supplements/mucu… iCoffee is a quack product and its marketing is based on flimsy evidence. There is no data of iCoffee improving diabetes or reducing weight. There is no data on its safety and side-effects in the short and long term. Please be aware of such products. I love coffee. Standard black coffee. But this iCoffee, I would not touch with a 10 ft pole. And please support doctors like @PremNarayanan9 who bring evidence-based medicine to the people at great personal cost. I hope he wins in Court.
