Heart and lung doses are low and comparable to international benchmarks. We report patient reported outcomes (PROs) using the EORTC-QLQ-C30 and QLQ ANL27 after 6-months follow-up. 706 patients were analysed (ACT3/4/5) with excellent PRO compliance. All treated groups experienced marked end of treatment declines with recovery by 6-months for most issues. ACT5 patients reported worse baseline PROs and persistently poorer bowel function at 6-months. At week 14, 76% of patients showed a good response, strongly correlating with MRI findings. Most were managed with watch-and-wait, achieving similar 5-year organ preservation rates to later assessments.
