"I sat in my supervisor’s office, red-faced and anxious, words tumbling out faster than I could control. For half an hour, I vented everything I had been holding in for months: the stress, the doubt, the sense that I didn’t belong. I was in the third year of my Ph.D., and a creeping fear had taken root that I wasn’t cut out for academia. "I expected some kind of judgment or disappointment. Instead, my supervisor listened patiently, then calmly offered a line I’ll never forget: 'You are here to learn to ride a bicycle, not to invent a bicycle.' That one sentence landed softly, but it cracked something open." scim.ag/44IGRd7 #NationalHigherEducationDay

Out in @EUplatinum: managing late GU adverse events after prostate radiotherapy requires a truly multidisciplinary approach. 🔑 Better collaboration = Less controversy = Better survivorship outcomes... 🔽 🔽 🔽 🔽 🔽 🔽 🔽 🔽 sciencedirect.com/science/ar…

Presented at #ASCO26: In high-risk localized prostate cancer, ADT plus apalutamide led to a pathological complete response or minimal residual disease and 5-year metastasis-free survival in a greater percentage of patients than ADT plus placebo. Early outcomes of partial gland (focal) salvage brachytherapy for radiorecurrent prostate cancer brachyjournal.com/article/S1… In a retrospective study of 100 patients with radiorecurrent #ProstateCancer treated with focal ☢️HDR or LDR brachytherapy, investigators found strong early disease control with low rates of severe toxicity. Three-year biochemical progression-free survival ranged from 50% to 84%, with presalvage PSA as the only significant predictor of outcome. No local failures occurred in the HDR2 or LDR cohorts, though follow-up remained limited. Overall, focal brachytherapy demonstrated efficacy and safety. @HimanshuNagarMD @seanmmcbride @marisakollmeier @OncoAlert 🚨 @Silke_Gillessen @AOmlin @weoncologists

BRCA1/2 may be telling us a bigger story. A new ESMO Open study suggests that pathogenic BRCA1/2 variants may increase the risk of less common cancers beyond breast, ovarian, pancreatic, and prostate cancer. Signals observed: BRCA1 → thyroid cancer BRCA2 → bladder, head & neck, and skin cancers The most striking finding: BRCA2-associated bladder cancer risk appeared particularly high in females. These data are hypothesis-generating, but they may reshape how we think about hereditary cancer predisposition and precision oncology. #BRCA #PrecisionOncology #CancerGenetics #Oncology @ESMO_Open @OncoAlert

Guideline of guidelines: lutetium-177 PSMA radioligand therapy in advanced prostate cancer bjui-journals.onlinelibrary.… Review synthesises international guidance on lutetium-177 PSMA radioligand therapy (177Lu-PSMA-RLT) in #ProstateCancer , focusing on metastatic castration-resistant disease. Major societies, including EAU, NCCN, ASCO, AUA/SUO, and CUA, endorse 177Lu-PSMA-617 after prior ARPI and taxane therapy, supported by VISION and TheraP trial data. Emerging evidence from PSMAfore is expanding use into earlier treatment settings. Current recommendations emphasise PSMA-positive patient selection, standardised dosing, androgen deprivation therapy continuation, and multidisciplinary management. @declangmurphy @DrMHofman @AzadOncology @RenuEapen @lawrentschuk @drMPerera @OncoAlert 🚨 @Silke_Gillessen @AOmlin @weoncologists

Current classifications of OMD assign one state per patient. But within the same patient, lesions respond, persist, and progress on divergent paths. We propose metastatic trajectories – a lesion-level framework to guide local systemic therapy in metastatic NSCLC. 📄 @JCO_ASCO

Local salvage therapies alone for #ProstateCancer recurrence after radiotherapy maintained >75% androgen deprivation therapy–free survival at 2 years with manageable rates of severe adverse events, supporting their use in selected patients. ja.ma/4dhVzMS

"Management of local prostate cancer (PCa) recurrence after primary radiotherapy (RT) remains challenging in daily practice. Salvage high-intensity focused ultrasound (S-HIFU) represents a valid option, but large prospective data are missing. 🚨HIFU for radiorecurrent #ProstateCancer🚨 HIFI-2 study in @EurUrolOncol 👬 Large prospective, cohort (n=531) 🇫🇷 Conducted across 41 French centers ⏱️ Primary endpoint: 30-mo ADT-free survival Results: 📊 30-mo ADT-free survival ~71% 🎯 Pre-HIFU PSA, earlier seems better 🚽 Trade-offs: severe incontinence (7%→12%) ⚠️ Complications low but not trivial (incl. fistula risk) But… ⏳ Only mid-term follow-up ❌ No comparator arm (RP, cryo, re-RT) 🧠 Selection bias evolving imaging practices 📉 ADT-FS ≠ oncologic cure @PCFnews @PCF_Science @AmerUrological @urotoday @UrologyTimes @GPloussard 🔗shorturl.at/q8zzu

Maybe working over Christmas wasn’t such a bad idea after all🤣 thanks @miszczyk_marcin 🚨Local treatments alone, for local recurrence of PCa after RT, can maintain disease control and postpone ADT🚨 Delighted to see DEFER study out in @JAMAOnc jamanetwork.com/journals/jam…

This is pretty big news from @UroOncJT and co @JUrology in press: MPS2 looks to be the first liquid biomarker offering real utility in the #activesurveillance setting for #prostatecancer. Much better NPV for upgrading than mpMRI - and less expensive! (in the US, anyway) auajournals.org/doi/10.1097/…

🎧 Music during prostate biopsy = real clinical impact 🚨VIVALDI RCT in @EurUrolOpen ✅Patient-selected music during transperineal prostate biopsy lead to.... ↓ Pain at every step (VAS consistently lower) ↓ Anxiety & distress post-procedure ↓ Sedation needs (37% vs 57%) ↑ Patient satisfaction & willingness to repeat 🫰Simple. Scalable. Zero downside. ⭐️So critical for anyone with a large Active Surveillance population 💡Nonpharmacologic interventions aren’t “soft”—they’re effective. 🔗shorturl.at/CslOk @PCFnews @PCF_Science @EUplatinum

Analysis of the CAP randomized trials demonstrates prostate cancer overdiagnosis is highly age dependent. Opportunistic screening policies have led to most PSA tests being in older men. These policies should be revaluated. onlinelibrary.wiley.com/doi/…
