Getting to the root of age-related diseases. By studying a rare accelerated aging genetic disorder, gain-of-function mutations of DNMT3A were found to be causal. The primary is the source of first progression in >50% of cases of #lungcancer patients, and TRACERx showed us that in 32% of NSCLC the primary polyclonally seeds metastases. In our @IASLC consensus, now in JTO shorturl.at/GzAe2, makes the case for eradicating it with #radiotherapy. The EGFRm phase III data are the most compelling OS 34.4 vs 26.2 months with TKI RT. The striking part isn’t just that a 14‑protein panel can flag lung cancer risk >5 years out, it’s that the signal is so IL‑1β‑centric. That neatly retrofits the CANTOS “accident” where canakinumab cut lung cancer incidence and mortality. We’re basically watching systemic inflammation turn the lung microenvironment pro‑tumor long before a nodule appears on CT. The next step isn’t more imaging, it’s immunoprevention trials in the high‑signature group.

Remarkable is an understatement! #ASCO26 7-yr update (Abstract 8502, CROWN): 1L lorlatinib in advanced ALK NSCLC — median PFS STILL not reached at 7 years, the longest ever in advanced NSCLC. 7-yr PFS 55% vs 3% for crizotinib. A new Nature paper shows how lung cancer metastases evolve and spread through the body. The study used sophisticated genomic analysis to reconstruct how one metastasis can seed another, and how the pattern of spread is shaped by anatomy, tumor evolution, and cancer biology.
