Broad Clinician Support – But Not Without Nuance 👨‍⚕️ Most HCPs supported SGLT2i use in T2DM with CKD or HF, even if foot ulcer history exists. ❗ No consensus was reached for active ulcer cases—highlighting a safety concern. ⚠️ 2. Canagliflozin is the Outlier 🚫 Canagliflozin should NOT be used in patients with prior ulcer or amputation. 📉 Risk of lower limb amputation (LEA) appears drug-specific, not class-wide. 🧠 3. Evidence-based Risk Reassurance 🧾 Multiple studies show SGLT2i reduce LEA risk compared to DPP-4 inhibitors and incretin-based therapies. 🩺 No significant LEA risk when compared to GLP-1RAs. 📌 The NICE (2024) guideline advises caution in PAD, ulcer, or LEA history—not absolute avoidance. 📦 4. Clinical Recommendations (From Delphi Panel) 💡 Prescribe SGLT2i in: T2DM patients with or without CKD/HF—even with prior ulcer/amputation history (🟡 exercise caution). ✅ Avoid canagliflozin in those at LEA risk. 🚫 Avoid in: 🟥 Active foot ulcer cases—due to lack of safety data. 🧾 Additional guidance: Continue SGLT2i in stable patients unless adverse effects emerge. Pathophysiological Insight on Canagliflozin Risk Potential reasons include: Volume depletion → ↓ tissue perfusion Changes in bilirubin metabolism Interaction with diuretics, smoking, hypertriglyceridemia ❗ Mechanisms are not fully understood, requiring further trials. 🧠 6.
